新診斷之愛滋感染者人數不斷增加,且大多數為青壯年。由於雞尾酒療法使得愛滋病患死亡率下降,平均餘命延長,然而在愛滋病的污名及慢性病程中,感染者的父母於照顧過程中面臨許多壓力。本研究的目的為探討成人愛滋感染者父母親之照顧經驗。藉由詮釋現象學的方法,還原愛滋感染者父母親的心路歷程,並詮釋父母親經驗之家庭壓力背後的深層意義。本研究以半結構式的訪談指引進行深度訪談,採立意取樣方式,選取7位成人愛滋感染者的父親或母親為研究對象,所得的文本以詮釋循環及詮釋螺旋的歷程,透過主題分析的方式,歸結成人愛滋病感染者父母親的照顧經驗為:「未料兒病惹爭議,只能假裝掩顧忌」、「羞憤痛苦受打擊,生活互動起變化」、「毋甘受苦頻叮嚀,盡心照顧盼轉機」、「無奈因果早註定,只盼為兒擔業障」四個主題。成人愛滋感染者父母親之照顧經驗最深層的生活經驗本質為「愛」與「責任」。本研究結果可增加臨床工作人員對於愛滋病家庭壓力經驗的整體性瞭解,而發展出以當事人經驗本質結構為導向的照護措施,並協助愛滋病家庭因應與調適疾病帶來的壓力。
The number of newly diagnosed patients with HIV is continuously growing and most patients are adults. The high active antiretroviral treatment dramatically reduces mortality and prolonged the life expectancy of patients. Parents of HIV/AIDS patients endure life-long distress while caring for their adult children. The study purpose was to elucidate the experiences of seven parents caring for adult children with HIV/AIDS. Hermeneutic Phenomenological methodology was applied to reveal the trajectory of parents’ caring experiences and meaning of caring experiences. Data were collected using semi-structured interviews. Interviews were tape-recorded and transcribed verbatim. Data were analyzed using thematic analysis with hermeneutic circle and Hermeneutic Spiral-Verstehen. Four themes were merged: (1) unexpected dispute provoked by their children’s illness causes parents to feign ignorance to conceal what is scrupled; (2) ashamed and resentful resulted from sufferings undergone and blows received; relationship and atmosphere changed; (3) parents feel protective toward them by giving advice repeatedly; hope there comes a chance of survival for their children by working laboriously ; and (4) having no way out of the predestined law of cause and effect, parents can only expect to bear for their children the burden of sin.Unconditional love and endless responsibility were the core experiences for these parents. The finding highlights the importance of patient-center nursing care for HIV/AIDS patients and for healthcare professionals to assist HIV affected families on related stress throughout the illness trajectory.
