本研究屬於探索性研究,因為台灣地區證據醫學(Evidence-based Medicine, EBM)的發展上在萌芽階段,因此以個案研究與內容分析方式來進行,並根據「立意抽樣」挑選彰化基督教醫院為研究對象,探討其證據醫學的推行狀況、證據醫學如何解決知識流通障礙、證據醫學對知識管理價值鏈的貢獻、以及組織間與組織內的知識流通效益。
研究結果發現:(1)大部分受訪者表示推行/使用證據醫學最主要的目的在於更新個人的知識,強調證據醫學對「個人」的重要性;(2)證據醫學推行的最大障礙在於時間的不足;(3)證據醫學提供一個良好的溝通機制或管道,可解決知識流通情境的障礙,但其他知識流通障礙的解決,證據醫學所能解決的程度有限,仍須組織的協助與承諾;(4)證據醫學可以提昇組織間與組織內的知識流通成效,尤其是個人與資料庫、同專科之間、與層級愈近者之間;(5)證據醫學的推行可促進知識管理價值鏈活動,包括知識創造、知識儲存、知識擴散、知識應用。另外,從學習型組織的角度來看,證據醫學的推行,對此實務觀念亦有相當大的貢獻。
本研究針對欲推行證據醫學此一機制的醫療單位,提出以下建言:(1)證據醫學要推行得好,必須要有高階管理者的支持;(2)完整的教育訓練計畫,可使組織內部人員清楚地瞭解證據醫學的真正意涵,也較能積極參與證據醫學活動;(3)完善的資訊系統可以讓醫療人員在存取資訊、知識時,更加容易;(4)適當的獎勵與懲罰可以激勵醫療人員投入證據醫學。
