本研究目的在探討影響台灣退化性膝關節炎之病患進行全膝關節置換術因子及未來老年人口的長期照護議題之研究。研究方法分為利用全民健保資料庫之次級資料進行前瞻性分析，以及利用現有資料比較各國長照現狀。透過自健保資料庫篩選出153名2001年1月被診斷為退化性膝關節炎之病患，追蹤其12年之就醫紀錄，將病患分為追蹤期內接受全膝關節置換術、以及未接受全膝關節置換術兩群體，並利用敘述性統計分析、卡方檢定、邏輯斯迴歸模型分析影響手術決策之因子，研究結果顯示年齡及性別對手術的意願皆有顯著影響。本研究結果可提供醫師建議退化性膝關節炎病患治療，及醫療及長期照護政策制定或執行之參考。

This study analyzes the factors of Osteoarthritis (OA) patients in Taiwan deciding to make Total Knee Replacement operation and elders’ long-term care issues in the nearly future. Data collected from National Health Insurance Research Database (NHIRD) is secondary data. Prospective Analysis is applied in this research. By selecting 153 patients who were diagnosed as Osteoarthritis in January 2001, then follow their treatment for OA for twelve years. Patients were divided into two groups, TKR and non-TKR. Moreover, Descriptive analysis, Chi-square analysis and Logistic regression model were applied to analyze the deciding factors. Results show that age and gender have significant influence on the willingness of receiving surgery. Practical implications from the research findings for OA patients, doctors and department of long-term care were discussed.
