失智症為心智能力下降,嚴重程度足以影響日常生活的總稱。其中,阿茲海默症是最常見的失智症類型,約佔整體失智症比例的六至七成。由於阿茲海默症為一種與年齡相關的神經退化性疾病,患者在罹患此症的初期容易被診斷為只是正常老化的現象。而不同的醫生對同一位病患的診斷結果也不盡相同。本論文透過同時觀察神經心理資料以及神經影像資料找出兩者之間的相關性,並透過統計假設檢定的驗證方式,加深實驗結果的可信度。我們從阿茲海默症腦神經影像計畫(ADNI)的數據庫中擷取31位由輕度知能障礙轉變至阿茲海默症的受試者作為實驗組,並另外擷取31位診斷前後皆為輕度知能障礙的受試者作為對照組。本論文主要的貢獻在於我們發現在診斷為輕度知能障礙轉變至阿茲海默症的群體中,左側與右側海馬迴子區域包含subiculum和presubiculum的體積(和表面積)與簡短智能測驗(MMSE)的細項分數定向力(ORI)之間存在正相關性並且具有統計顯著性(p<0.05)。然而,在兩次診斷皆為輕度知能障礙的群體中並未觀察到這些相關性。希望透過這一項發現降低病患在確診前因為侵入式的檢測造成身體上的不適,並協助醫生早期診斷病情,使得醫療資源可以被更有效率的運用。
Dementia is a general term for the decline in mental capacity, which is serious enough to affect daily life. Alzheimer’s disease is the most common type of dementia, which accounts for about 60 to 70% of dementia. Because Alzheimer’s disease is an age-related neurodegenerative disease, patients are easy to be diagnosed as normal aging at the early stage of the disease. Different doctors may also have different diagnostic results for the same patient. In this thesis, we observe the neuropsychological data and the neuroimaging data simultaneously and find out the correlations among them. We also deepen the credibility of the experimental results through the verification of statistical hypothesis testing. We took 31 subjects from mild cognitive impairment to Alzheimer’s disease in the dataset of Alzheimer’s Disease Neuroimaging Initiative (ADNI) as the experimental group and took 31 MCI non-converter subjects as the control group. The main contribution of this thesis is that we discovered positive correlations and statistical significance between the MMSE sub-item orientation score and volume (and surface area) of sub-regions including subiculum and presubiculum for both the left and right hippocampus in the MCI to AD cohort. However, these correlations were not observed within the MCI to MCI cohort. We hope that through this discovery, patients will be relieved of physical discomfort due to invasive testing before definite diagnosis, and assist medical doctors to diagnose at the early stage, so that medical resources can be used more efficiently.
