傳統中國醫學是亞洲地區前人最偉大的智慧與寶貴的文化資產,亦是歷史中數千年來與疾病對抗所留下的珍貴經驗法則。中醫學以整體觀和平衡觀為核心,建立了獨到的生命理論與診療體系。中醫的診斷包含望、聞、問、切四診,切診當中以脈診最好,脈診是由外向內偵測患者身體狀態的方法,但其技術與經驗的傳承欠缺了科學化的輔佐。
在傳統中國醫學中,脈診儘管居於四診之末,但卻佔有重要的一席之地,透過脈診可以得知脈象,而不同的脈象反應不同的生理訊息。近年來許多研究利用單點式手腕橈動脈之脈波波形分析作為病理的解析,並區分中醫脈象,但因為單軌波形無法呈現空間資訊,導致與手指之感覺有相當的差異。因此,本論文使用陣列式感測脈診儀取得手腕橈動脈之壓力波波形,並透過數值分析建立可視化脈波圖譜以呈現脈象。
本論文於受測者左手腕關部透過冷刺激的方式來使得區段的血管順應性產生變化,並且全程觀察其壓力波形在基態期、刺激期以及恢復期的變化。基態期時,會先量測60秒的橈動脈脈波波形並量測手腕尺部溫度,此段時間為Base階段;接著,刺激期時,會將事先準備好的冰袋放在手腕橈動脈上,並且每40秒會量測一次橈動脈壓力波形以及其溫度變化,總共量測三次,這三次分別稱為C1、C2與C3,刺激期時間共為120秒;最後,在恢復期時,將冰袋拿掉,並且每40秒量測一次橈動脈壓力波波形及其溫度變化,共三個階段,分別稱為R1、R2與R3,恢復期的時間總共120秒。整個實驗流程時間共為5分鐘,並且共有7個階段,分別為Base、C1、C2、C3、R1、R2與R3。
收集到的資料將會製作成脈波圖譜的形式,並且透過等高線圖來分析空間波形,與此同時,定義脈波圖譜的各項參數;參數再經過初步的統計分析後,脈波圖譜的參數會被導入三角形歸屬函數中,藉由調整參數和歸屬函數之間的權重來設計脈波圖譜的模糊專家系統。
本論文針對30名健康受測者男性18位,女性12位,年齡為22.10 ± 3.52歲、BMI 20.30 ± 1.54、收縮壓 111.30 ± 4.96、舒張壓73.57 ± 3.31;經局部冷刺激使得手腕橈動脈血管順應性改變後,發現脈搏波形分析中的增大指標與H3/H1會變大(P<0.001);3DPM等高線分析中,等高線為0.5至0.8的長軸顯著地變長(P<0.001);Dynamic 2DPM等高線分析中,長軸也會顯著地變長(等高線值為0.5至0.8時,P<0.001)、時間軸也會顯著地變長(等高線值為0.5與0.6時,P<0.001;等高線值為0.7時,P<0.01;等高線值為0.8時,P<0.05)。將統計分析後的參數範圍設計而成的模糊專家系統,其分辨出基態期有15%的受測者呈現緩脈、65%亞弦脈、20%弦脈,冷刺激的過程中有 0%的受測者呈現緩脈、10%亞弦脈、90%弦脈,恢復期有 12%的受測者呈現緩脈、30%亞弦脈、58%弦脈。
本論文證明冷刺激會使得血管順應性變差、血壓會升高,且符合中醫理論中「高血壓弦脈」的現象,其能夠將血管的改變以視覺化的方式描述,醫師亦能更精確判讀和掌握患者病情,對廣大的民眾是一大福音;再者,利用模糊專家系統可以自動分類脈象,可供醫師決策時有參考的依據。
Traditional Chinese medicine is the one of great wisdom and experience of culture in Asian; it protects Asian health for thousands of years. Chinese medicine is not as Western medicine, the core value of Chinese medicine is the overall concept and the concept of balance, creates unique life theory, diagnosis system. In clinical diagnosis, Chinese medicine physicians use four methods: inspection, listening and smelling, inquiring, and palpation to diagnose diseases. However, Chinese medicine lack of scientific verification, assistant; otherwise, Western medicine use anatomy, physiology to create scientific model so that could focus on target diseases and make more efficiency.
This thesis utilized multiple-point array sensor to record radial pulse waves, and then constructed pulse mapping with numerical analysis. 30 healthy subjects, 22.10±3.52 years old, BMI 20.30±1.54, and blood pressure 111.30±4.96/73.57±3.31mmHg, were studied by given local cold stimulation test at “Guan” on left hand. This thesis utilized multiple-point array sensor to record radial pulse waves, and then constructed pulse mapping with numerical analysis. 30 healthy subjects, 22.10±3.52 years old, BMI 20.30±1.54, and blood pressure 111.30±4.96/73.57±3.31mmHg. Each participant was studied by a local cold pressor test to induce anomalous pulse waveform. After 60 seconds of seated rest, radial pulse waves were recorded named as base stage. Then, an ice bag was put on the wrist, avoiding the measured point, and both radial pulse waves and the temperature of stimulated point were recorded every 40 seconds for 120 seconds named as stimulation stage. Lastly, the ice bag was taken off, and both radial pulse waves and temperature were recorded every 40 seconds for 120 seconds named as recovery stage. It took five minutes for the local cold pressor test. The pulse data is collected by radial pulse acquisition system, using contour analysis to define the parameters within pulse mapping, and then the parameters are designed as a fuzzy expert system using triangle membership function. Adjusting weighting from the relation between the membership function and parameters could make more optimize fuzzy expert system.
According to results, on PWA, significantly higher (P<0.001) AI and H3/H1 occurred after cold pressor test making radial artery stiffer. On pulse mapping analysis, longtinudal axes were significantly longer as contour values were from 0.5 to 0.8 (P<0.001) and so were in time axes (P<0.001 as contour values were 0.5 and 0.6; P<0.01 as contour values were 0.7; P<0.05 as contour values were 0.8). This implied that cold pressor test induced lower arterial compliance associated with string-like pulse and provided physicians with a novel way to take care of patients’ health status. The fuzzy system suggestion 15% as normal pulse, 65% slightly string-like pulse and 20% into string-like pulse.
This implied that local cold pressor test induced lower arterial compliance, high blood pressure associated with hyptertension type string-like pulse in traditional Chinese medicine. Furthermore, this finding could be provided that physicians with a novel way to classify the pulse type, take care of patients’ health status.
