  • 背景與目的:
    精神疾病烙印(stigma)為針對精神疾病患者之過於籠統與不合理的負面態度或行為。精神醫療及健康專業人員以促進患者之健康及安適感為職責,然而對精神疾病之烙印源於社會及醫療文化,因此精神健康照護專業之工作人員亦可能存有烙印態度。若精神健康照護工作者未意識到自身對精神疾病的烙印,並在與患者互動中不自覺地流露,將可能導致不佳的醫病關係或治療預後,故探討精神健康照護工作者對精神疾病之烙印有其重要性,然而,在臺灣相關議題之研究及討論仍顯缺乏。本研究以臺灣精神領域的職能治療實習學生與專業人員為對象,探討其對精神疾病之烙印,並進一步檢驗烙印之相關與預測因子。

    方法:
    本研究為橫斷型研究,透過設有職能治療學系之大專院校,招募一年內於精神領域實習滿12週之職能治療學生,亦透過社團法人中華民國職能治療師公會全國聯合會及各地區之職能治療公會,招募持有有效執照登記,且於精神領域工作之職能治療人員。本研究使用網路自評問卷,內容包含人口學特徵、健康照護工作者開放心態量表、十項人格特質量表、奧爾登堡職業倦怠量表以及短版社會讚許量表之項目,並對資料進行描述性統計、獨立樣本t 檢定、單因子獨立變異數分析、相關性分析與迴歸分析。

    結果:
    本研究共納入62名學生及151名專業人員。研究發現,具碩士學歷的專業人員其烙印顯著低於學士學歷者;而與社區及門診場域相比,住院場域的專業人員表現出更高的烙印。在人格特質方面,專業人員的高親和性與高情緒穩定性皆與較低的烙印相關。此外,不論是學生或專業人員,職業倦怠程度越高,其烙印也越深,其中學生的烙印則主要與實習期間的精疲力竭相關,而專業人員的烙印與工作疏離具較強之關聯性。本研究進一步以迴歸分析發現,學生的烙印受其實習期間的精疲力竭狀況所預測,而專業人員的烙印可由其工作疏離、親和性與自我欺瞞程度所預測。

    結論:
    本研究結果發現,臺灣精神領域職能治療實習學生之職業倦怠,以及專業人員之學歷、執業場域、人格特質與職業倦怠,為其對精神疾病烙印之重要相關與預測因子。研究建議,教育單位應於實習階段關注精疲力竭與態度養成;臨床單位則需提供足夠資源支持專業人員,注意其對工作之疏離感受及親和性特質,並促進對烙印之覺察。研究結果也將作為臺灣各精神健康照護專業之參考,期待藉以引發各專業對自身烙印態度之反思,進而促進精神疾病患者之照護品質與權益。

    This study investigated stigmatizing attitudes towards mental illness among psychiatric occupational therapists and students in Taiwan. Mental health professionals may inadvertently harbor biases, which may undermine therapeutic relationships and treatment outcomes. This cross-sectional study used self-administered online questionnaires, including demographic data and validated scales, to explore factors associated with and predictors of stigmatizing attitudes. Results showed that educational level, practice setting, personality traits, and burnout were key factors. Master's-level professionals and those in community-based and outpatient settings held less stigmatizing attitudes. Higher burnout, particularly disengagement for professionals and exhaustion for students, predicted higher stigma. Among professionals, agreeableness and self-deception also emerged as significant predictors. The findings emphasize the importance of addressing both personal and contextual factors to improve attitudes and enhance the quality of psychiatric care. The study further encourages critical reflection within the mental health community and supports the promotion of rights and occupational justice for individuals with mental illness in Taiwan.
