研究背景:
全球高血壓盛行率不斷攀升,顯示高血壓未能良好控制。慢性病管理為公共衛生保健的重要課題,高血壓為多項慢性疾病之危險因子,其併發症可能造成更嚴重之健康負擔。高血壓管理包含眾多重要因素,如飲食型態、鹽分攝取、規律運動及持守(藥物服用,方能降低血管損傷及相關慢性疾病風險。然,血壓控制問題因病人持守(medication adherence)不佳未能達到良好控制,全球高血壓控制率僅約兩成。跨理論模式(Transtheoretical Model, TTM)應用於行為改變已被證實能促進慢性病病人之健康行為改變與持守,並有助於建立長期健康生活型態,進一步探究應用重要關鍵人幫忙關係介入措施於其他慢性疾病控制之成效。
研究目的:
運用重要關鍵人幫忙關係策略於高血壓病人健康生活型態、服藥持守、血壓變化及幫忙關係之成效。
研究設計:
本研究採隨機分派二組前後測研究設計 (randomized two-group pretest-posttest design),以亂數表隨機分派為實驗組及對照組,介入常規高血壓健康生活型態衛教,實驗組則加上重要關鍵人幫忙策略指導,比較兩組間在重要關鍵人幫忙關係介入前後,健康促進生活型態、服藥持守、血壓數值以及高血壓病人所感受幫忙關係之影響,本研究因介入性質無法進行盲化。在台灣南部醫學中心門診進行收案,兩組皆完成前測並追蹤三個月,實驗組每周追蹤執行狀況,採立意取樣,收案條件為:大於十八歲、診斷高血壓、高血壓病人具有重要關鍵人者、意識清楚可以國台語溝通者、同意參加本研究者。資料以SPSS 進行分析,人口學資料會以描述性統計分析;推論性統計分析會先以獨立樣本t檢定及χ² 檢定進行兩組資料基線比較,依資料樣態採用共變數分析或變異數分析,藉此瞭解幫忙關係策略介入後成效。
研究結果:
研究最終納入分析樣本為133對,結果顯示兩組於介入前之基本人口學資料及主要結果變項多具有同質性,經三個月介入後,實驗組於血壓、健康促進生活型態、服藥持守及所感受之幫忙關係等結果變項之變化,與對照組相較多未達統計上顯著差異。幫忙關係策略介入對多數量化結果之短期效果有限,可能與研究對象多為高齡族群、介入時間較短,以及部分變項基線狀態已相對穩定有關。質性資料顯示,重要關鍵人透過幫忙關係策略,協助病人維持服藥、量測血壓及調整生活型態,顯示幫忙關係策略在促進自我管理行為與維持健康行為穩定上具有臨床應用潛力。
結論:
運用重要關鍵人幫忙關係於高血壓病人健康生活型態、服藥持守、血壓變化及幫忙關係程度有正向趨勢影響,雖未達統計上顯著,電訪追蹤資料則顯示幫忙關係仍有助益,建議未來可應用於慢性病長期管理,補足目前依從性不佳問題。
This study examined the effects of helping relationship strategies involving significant others on blood pressure control, medication adherence, health-promoting lifestyle, and perceived helping relationships among patients with hypertension. A randomized two-group pretest-posttest design (open-label randomized controlled trial) was adopted. The experimental group received a three-month helping relationship intervention, while the control group received usual care. Although the differences between groups did not reach statistical significance, the findings suggest that the helping relationship strategy may support behavioral change and enhance interpersonal support in hypertension management. The results indicate that involving significant others may play a meaningful role in chronic disease self-management and highlight the potential value of relationship-based interventions in hypertension care.
