近年來，超音波水晶體乳化術（Phacoemulsification；PE、Phaco）併植入人工水晶體已成為白內障治療方式的主流趨勢，特殊功能人工水晶體（Functional Intraocular Lens）由於可降低一般功能人工水晶體（monofocal Intraocular Lens）引起的復發比率（Relapse rate），且生物相容性（Biocompatibility）更佳，但健保對特殊功能人工水晶體採用部分給付制度，故本文想探討影響白內障患者置換特殊功能人工水晶體因素分析。
本研究架構以Fishbein與Ajzen（1975）的理性行為理論（Theory of Reasoned Action，TRA）為基礎，探討相關行為主觀規範、態度及行為意向對病患的選用特殊功能人工水晶體的行為分析，另外加上社會人口學相關因素，以探討這些變數因素是否對此醫療行為有顯著影響。
本研究問卷問項共分四部份，分別為行為態度、主觀規範、行為意圖及人口統計學變項，經以問卷調查方式調查200位病患，共回收有效問卷178份，其有效問卷回收率89%；並運用EViews等相關統計軟體進行相關如線性機率模型（Linear Probability Model, LPM）、Ｔ檢定等相關檢定分析，以建立迴歸方程式模型並討論之。
其研究結果如下：(1)驗證主觀規範、態度、行為意圖及其他病史變數均有顯著關係；在主觀規範變數中以「家人的支持與朋友介紹」有顯著關係，而在態度變數中「視覺品質對於未來生活期待與特殊功能人工水晶體價格昂貴品質比較好」有顯著關係，另在行為意圖變數中顯示對於「工作與休閒需求」有顯著關係，因此推論這些因素影響相關白內障病患對選擇置換特殊功能人工水晶體的行為。(2)驗證態度變數中「醫護人員的建議」與行為意圖變數中「支持主治醫師選擇的特殊功能人工水晶體」呈現不顯著關係；也顯示相關病患在面臨醫療需求時，醫護人員的專業判斷等相關變數因素在本研究中顯示不一定會影響病患本身的決定。

In recent years phacoemulsification with intraocular lens implantation has become a mainstream treatment for cataract. Functional intraocular lens outweighs monofocal intraocular lens due to its lower relapse rate and better biocompatibility. However, the National Health Insurance (NHI) only adopts the co-payment policy for functional intraocular lenses. Thus, in this study we intended to analyze the possible influential factors of patients with cataract on their choices of functional intraocular lenses. Based on the Theory of Reasoned Action (TRA) proposed by Martin Fishbein and Icek Ajzen in 1975, this study investigated the relationships among attitudes, subjective norm, intention and behaviors in choosing functional intraocular lens and examined if these factors, along with socio-demographic factors, had a significant influence on the medical behavior. The questionnaire of this study consisted of four sections: attitude, subject norm, behavior intention and demographic variables. A total of 200 questionnaires were released to the patients with cataract, and 178 valid questionnaires were enrolled, with the valid return rate of 89%. Statistical software (Eviews) was employed in the study to conduct test analysis including Linear Probability Model (LPM), T-test, and regression equation model. The results of this study show some characteristics attribute the decision making among the different categories: “family support and friends’ recommendation” in the category of subjective norm , and attitude category, “visual quality for future expectation and expensive functional intraocular with better quality” means significant , about “work and leisure demands” when consider about the category of behavior intention and other medical history. Hence, it was concluded that the decision making of cataract extraction with multifocal intraocular lens implantation to those patient with cataract. “family support and friends’ recommendation” , “visual quality for future expectation and expensive functional intraocular with better quality” and “work and leisure demands” are the most contributing factors. . This also implies that cataract patients’ decision-making is not affected by medical staff’s professional judgments when facing medical demands.
