近年來，由於國內醫療產業經營型態的轉變，使得以往具獨佔特性的醫療院所亦面臨到競爭的壓力。因此，如何提高醫院經營管理的績效已成為各級醫院管理階層所共同關心的焦點。特別是藉由資訊科技的應用來提昇競爭優勢已成為目前醫院發展的主要目標之一。隨著資訊與網路技術的發展、儲存媒介成本的降低、及影像處理技術的成熟，使得數位化醫學影像儲傳系統(PACS)的建置，已成為國內各級醫院資訊系統發展的重要目標。目前國內雖有成功建置PACS的醫院，但亦有許多失敗的案例，同時因建置全院性之PACS必須投入相當大筆之費用，以致於國內多數醫院對於PACS的發展仍停留在觀望階段。
本研究主要內容為醫學影像儲傳系統之規劃與管理及主要之應用。首先在系統規劃上，從系統作業流程之探討與說明、系統基本架構設計、硬體設備需求規劃、軟體功能規劃、資料庫系統設計、資訊系統之整合規劃至系統之安全性與穩定度之規劃等，對系統做一整體性之探討與規劃。其次在系統管理方面則著重於系統資料庫之安全管理、影像儲存與傳輸管理、網路品質與安全管理、網路儲存媒體之管理、與整合照顧病患之品質管理。最後探討本研究之規劃所產生之效益與經濟效益分析，另外說明PACS在遠距醫療與電子病歷及健保申報之應用中所扮演之角色。

Lately because of the changes of healthcare industry ecology, hospitals which monopolized the healthcare market in the past and now they have encountered great competing pressure. How to enhance management strategies and raise the profit are the key issues for many hospital administrators. To promote healthcare services strength by information technology for hospital management is one of the principal approaches. With the development of computer and network technology, degradation of mass storage costing, and the mature technique of medical many hospitals are trying to apply the Picture Archiving and Communication Systems (PACS) as a part of the Hospital Information System (HIS). Although there are some successful cases, but some are failures. Since the investment of establishing a fully PACS is enormously, many hospitals are still keeping hold of hesitation.
The purpose of this study is to plan a Picture Archiving and Communication System for a large-scale hospital. Considerations in both technique and management are studied. System planning includes system operation flow, structure design, specification of hardware requirements and software functions, database design, integration of information system, system stability and security. System management emphasizes on database security, the management of image storage and transferring, networks quality and security control, remote mass storage media management, and the integration of the patient care. The benefit and economic effect of this system were analyzed. Related applications of the PACS in Electronic Patient Record (EPR) and telemedicine and healthcare Pre-examination Operation were also discussed.
