  • 研究目的:因神經行為科學之蓬勃發展,心理治療之理論與技巧隨之改變,治療取向轉以大腦功能為基礎,稱之神經心理治療(Neuropsychotherapy),為目前臨床心理學新興之治療模式,過去僅有提出相關部份病理方向,其治療內涵在臨床上尚未有完整之共識。故本研究結合本土臨床心理師應用之經驗觀點與實證性研究來探究神經心理治療之內涵建構。
    方法:本研究共分成三個階段。第一階段為質性研究,由九位臨床心理師或儲備者以神經心理學理論與心理治療為討論議題,進行十八次焦點團體,並紀錄個別參與者對神經心理治療之理論重點與技術省思,共得143份資料進行扎根理論分析,建構出初步神經心理治療內涵。第二階段進行專家會議檢驗此內涵建構之合宜度。最後進行開放性與實證性研究之文獻整理,以支持此理論架構的適當性。
    結果:本研究綜合第一部份和第二部份結果呈現神經心理治療內涵有三大主軸,包括:病理軸、治療目標軸,以及治療策略軸。「病理軸」分成兩個主要面向,共十一個項次;而「治療目標軸」分成兩個主要面向,共十一個項次;最後「治療策略軸」分成兩個主要面向,共十三個項次。第三部份研究結果發現,至2012年止,超過九成五的項次皆已有參考性的實證文獻。
    討論:研究結果顯示,神經心理治療已有豐富的內涵和足為依據的臨床研究。整個架構呈現出以下三種特色,其一:病理角度上更著重跨診斷觀點和大腦認知功能;其二:以了解個案整體心智功能部件之運作和同時增強治療情境下相關人之心智部件作為主要介入目標;最後,依大腦心智功能特色來創新、運用或融合多元治療策略,並重視對治療者前額葉監控功能之強化。本研究雖為先趨研究之本質,但足以展現神經心理治療之內涵架構。

    Purpose: The significant advancements in neuroscience have brought psychotherapy into a new approach, Neuropsychotherapy, of which the focus will be on the functional level of the brain. However, consensus has not been reached on this model. This present study aims to investigate the framework and conceptual model of Neuropsychotherapy with both empirical research and clinical experience.
    Method: There are three parts in this study. Qualitative method was used on the first part. 143 data has been collected and analyzed with Grounded theory from 18 times focused-groups, which were set up by 9 clinical psychologists and reserves, discussing on the issues between psychotherapy and neuropsychology. Data was included the essential of Neuropsychotherapy and skills reflection, and the basic model of Neuropsychotherapy was built up. Second, conduct expert meeting to examine the fitness of the model. The theoretical framework will be supported by relevant empirical literature evidences.
    Result: According to the first two parts result, there are three main axials in this model, including: “pathological view”, “therapeutic goal”, and “therapeutic strategy”. There are two aspects together with eleven themes in the “pathological view”; while there are two aspects, eleven themes in “therapeutic goal” and two aspects, thirteen themes in “therapeutic strategy”. The findings of the third part showed that more than 95% items had empirical references consistent with this model until 2012.
    Discussion: This model has three main features. First, It emphasizes more in trans-diagnostic approach and the mental functions. Second, it aims to strengthen specific mental function during the therapy sessions by evaluating patient’s whole profile of mental functions. Lastly, various therapeutic strategies will be employed, merged and reviewed according to the way that how the brain works. The improvement of psychotherapist’s self-monitoring of prefrontal lobe function will also be valued. Although it is a pilot study, it still demonstrates the framework of Neuropsychotherapy.
