  • 研究背景:原發性腦腫瘤雖相對罕見,卻逐年增加,且因治療進步增加存活率,讓腦瘤患者的生活品質受到重視,患者有機會透過復健,改善受損的功能。原發性腦瘤病患的認知功能受損問題會影響病患的生活品質和預後,造成病患及其照顧者生活中極大的困擾,過去治療的焦點著重治療的過程或處理生理功能方面的問題,病患照顧者認知照護能力與病患認知功能影響的實證研究付之闕如。
    研究目的:本研究透過問卷了解原發性腦瘤病患認知功能的現況和原發性腦腫瘤病患主要照顧者的照護能力,進一步探討原發性腦腫瘤病患主要照顧者的認知照護能力對原發性腦瘤病患認知功能的影響。
    研究方法:本研究採相關性研究設計,透過橫斷式研究(Cross-sectional research),以方便取樣方式選取南部某醫學中心神經外科門診和病房病患和家屬作為研究對象,透過問卷收集包括基本資料、蒙特利爾認知評估(The Montreal Cognitive Assessment, MoCA)、照顧者能力評估表收集資料。
    研究結果:研究共納入42對病患與主要照顧者,病患MoCA分數平均為23.57(±5.81),分數大於等於24分者佔66.67%;照護者照護能力分數差距大,最低為21分,最高為80分,平均為64.71;原發性腦瘤病患主要照顧者對於認知障礙在資訊收集方式、處理經驗與分享能力的部分較低;病患年齡、病患教育程度、腫瘤大小、主要照顧者照護時數、主要照顧者照顧型態與病患認知功能有關(p<.05)。分層羅輯式回歸分析結果顯示,腫瘤大於等於三公分之病患,主要照顧者認知照護能力影響病患認知功能(p=0.01)。
    結論:腦瘤病患主要照顧者對腦瘤病患認知改變的識別能力不足,未來可以增進臨床上醫療人員對於主要照顧者認知功能相關資訊的提供,特別是腫瘤較大患者之主要照顧者,進而協助主要照顧者提供病患居家認知復健活動,增進患者和其主要照顧者對疾病的調適。

    Primary brain tumors are relatively rare tumors, and increased gradually. Most health professionals usually focus on treatments to improve physiological function. In addition, research on cognitive care ability of caregivers and the impact to patients' cognitive function is lacking. The purpose of this study was to understand the impact of the cognitive care ability of caregivers on cognitive function of patients with primary brain tumors. We used correlation, cross-sectional and convenience sampling to investigate the status of cognitive function in primary brain tumor patients in this study. The data analyzes by chi- square test, linear regression and logistic regression. Forty-two patients and their caregivers were recruited. Caregivers have lower levels of information collection, processing experience, and lack of ability on sharing experience about patients’ cognitive impairment. Patient’s age, patient’s education, tumor size, caregiver's care hours and caregiver's type of care were related to the patient's cognitive functions. As for the patients with tumor size larger than 3cm, their caregivers’ cognitive care ability affected patients’ cognitive function. In the future, providing caregivers information about cognitive function in clinical scenarios is suggested.
