傳統醫學教育的初期教學主要以書面資料及口頭授課為主,然而許多臨床技能需要透過實際的操作以累積經驗、逐漸熟練。目前學生時常缺乏實際演練的機會,進而造成學校教育與臨床實務銜接困難,成為師生雙方的負擔。以「美國國家衛生研究院腦中風評估量表 (NIHSS)」為例,該量表為急性腦中風病患入院時皆須進行的嚴重度評估可信量表,共有11項指標,熟練的醫師可以在5分鐘內完成評估,然而不熟練的評估者在巨大的壓力下,可能造成評估時間延長或誤判。
近年來虛擬實境(Virtual Reality, VR)在醫學培訓領域中逐漸嶄露頭角。本研究透過虛擬實境技術開發一款「NIHSS中風評估虛擬實境模擬培訓系統——SS+」,創造一個模擬練習的空間,以提供醫學生臨床實作前一個演練的機會,藉由實際操作來熟悉繁瑣的流程並累積評估經驗。透過真實病例個案的導入,學生可以在虛擬環境中進行全方面學習,包含評估流程、與病人互動的技巧、病人表現的觀察要點與給分準則。這種方式相對於傳統課程提供了更直觀且實際的學習體驗,使學生更容易理解並記憶相關知識和技巧。學生在SS+中模擬練習後才進入臨床實踐,能讓他們能夠自信準確地判斷病人中風嚴重程度,並給予適當且即時的醫療處置。
為驗證這個虛擬實境培訓系統的有效性,我們設計了一個包含前後測的實驗,比對受試學生在VR學習介入前後的表現,並填答虛擬空間存在感、系統易用性,以及情境學習動機等三個已知問卷。此外,研究團隊也進行了深入的訪談,以瞭解使用者對於虛擬實境培訓的感受和看法。這些訪談提供有價值的質性資料,揭示虛擬實境培訓對於醫學生和臨床醫師的實際影響。
綜上所述,我們不僅在技術層面上開發了一個具體的培訓系統,也透過實驗全面評估了虛擬實境在醫學培訓中的應用效果。目前成果已實際進入教學課程中,透過使用者研究了解虛擬實境在緊急醫療教育中的價值,盼能作為未來發展的參考。
Traditional medical education has predominantly relied on didactic teaching in the early stages, with hands-on clinical practice occurring later during hospital internships. However, many clinical skills require accumulated experience and hands-on practice for proficiency. Traditional didactic training methods lack interactivity, and learners often have limited opportunities for hands-on practice and experiential learning.
Taking the example of the "National Institutes of Health Stroke Scale (NIHSS)," a widely used assessment tool for acute stroke patients, consists of 11 assessment indicators that require practical experience for accurate evaluation. Proficient physicians can complete the assessment within 5 minutes, but inexperienced assessors under significant time pressure may extend the evaluation time or make errors, potentially impacting patient outcomes negatively.
Traditional medical education often leaves students lacking practical training opportunities, creating challenges in bridging the gap between academic education and clinical practice. Recognizing these challenges, this study aims to develop a "NIHSS Stroke Assessment Virtual Reality Training System." Leveraging virtual reality technology, this system provides medical students with simulated practice opportunities for stroke assessment before engaging in clinical practice.
Through hands-on experience in a virtual environment, learners can familiarize themselves with the intricate assessment process, enhancing the comprehensiveness of their training and ultimately contributing to the improvement of healthcare service quality. Incorporating real patient case scenarios allows learners to engage in comprehensive learning, including the assessment process for acute strokes, critical observation points for patient performance, interaction skills, severity rating criteria, and self-assessment of learning outcomes. This system has been applied in medical courses, and user studies, experiments, and interviews were conducted to thoroughly analyze the value of virtual reality in emergency medical education and identify directions for future improvements.
