  • 摘要
      根據偵測作業及叫色作業文獻指出,焦慮的個體對於威脅刺激的偵測較敏感,也較容易受到威脅刺激的內容干擾而產生注意力的偏差。近年來,研究負向促發作業的學者則提出焦慮者的「抑制缺損假設」,認為焦慮的個體對於干擾刺激的注意抑制能力可能較正常人差。本研究使用負向促發作業探討焦慮症患者對於干擾刺激的注意抑制及記憶提取歷程,並同時探討焦慮症患者對於威脅刺激的不同反應模式。
      研究對象為台北市立療養院未使用藥物治療之門診焦慮症與強迫症患者,針對每位參與者,皆施測由陌生情緒面孔(中性及威脅)所組成之負向促發作業,評估個體對於不同實驗操弄情境之反應時間;本研究另外以「情境-特質焦慮量表(STAI)」評估受試者的焦慮狀態與焦慮特質程度。
    反應時間以ANOVA進行檢定分析,促發項的反應結果顯示,強迫症患者反應時間在威脅干擾情境中顯著較中性干擾情境大,而正常人與其他焦慮症患者則無此顯著傾向,顯示強迫症患者受到較多威脅刺激的干擾效應。偵測項的反應結果則顯示,正常人呈現顯著的中性及威脅負向促發效應,強迫症患者的中性及威脅負向促發效應消失,而焦慮症患者則未產生中性負向促發效應,但卻呈現顯著的威脅負向促發效應;另一方面,正常人的中性負向促發顯著比焦慮症及強迫症患者大,而正常人與焦慮症患者的威脅負向促發則均大於強迫症患者。
    強迫症與其他焦慮症患者於促發項及偵測項中呈現不同的反應模式,焦慮個體的注意力「抑制缺損假設」得到驗證,強迫症患者呈現普遍性抑制功能缺損,其原因可能來自其額葉功能問題,而其他焦慮症患者對於威脅刺激呈現不同的反應模式,則可能來自「認知逃避」或「記憶提取」作用,因此,強迫症與其他焦慮症可能源自不同的病理機制而呈現不同的表現模式。
