背景:
台灣已邁入超高齡社會,且高齡者就診高年精神門診與去日間機構參與照顧和治療的人數逐年增加,過去研究發現,音樂介入對失智患者的情緒、認知功能和生活品質可能有改善的效果,但多以安養機構的失智患者為團體介入的對象,國內仍較缺乏日間機構的相關研究,因此本研究的目的在探討音樂介入對老人日間機構個案的影響及適用性。
方法:
本研究以台灣南部兩個老人日間機構為收案地點,納入條件為臨床失智評估量表評分需≧0.5,並排除認知功能退化而無法切題回應或無法參與音樂介入之對象。介入方式為每周一次,每次60分鐘的八次音樂性治療活動,採單組前後測設計,測量工具包含:康乃爾失智者憂鬱量表中文版、認知功能障礙篩檢量表以及失智症生活品質量表,並在最後一次治療活動以滿意度問卷和團體討論收集個案對接受音樂介入之感受,以探討此介入的適用性。
結果:
本研究共招募40位受試者,結果顯示在介入前後,受試者在情緒、認知功能和生活品質無顯著改變,但對生活品質的心情分量表呈現小到中的效果量。團體滿意度高,所有受試者皆認為音樂介入對自己的情緒、認知功能和生活品質有所幫助,且適合自己參加。
結論:
本篇研究發現雖然音樂介入後對日間機構老人在專業人員評量之情緒、認知功能和生活品質沒有顯著效果,但個案自評量表之結果呈現對此介入滿意且認為介入對自己有益。此音樂介入可適用於日間機構的老人族群,建議未來研究可挑選更敏感的評估工具,增加收案人數,並透過對照組確認介入的效果。
Music interventions have shown promise for improving mood and cognition in dementia, but research in Taiwanese day care centers is limited. This study investigated the effects and feasibility of music interventions for older adults in these centers.
Forty participants from two southern Taiwanese day care centers received eight weekly 60-minute music therapy sessions. A pre-post design was used, with assessments using the Cornell Scale for Depression in Dementia (CSDD), Cognitive Assessment Screening Instrument (CASI), and Quality of Life in Alzheimer's Disease (QoL-AD) scales. Participant satisfaction was also assessed.
Participants reported high satisfaction and perceived benefits in mood, cognition, and quality of life. No statistically significant improvements were found on the CSDD, CASI, or overall QoL-AD scores. A small to medium effect size was observed on the mood subscale of QoL-AD.
Although professional assessments did not demonstrate significant changes, the positive participant feedback suggests potential benefits of music interventions in day care centers.
