本研究旨在探討總額預算制度實施後對牙醫師醫療行為的影響，研究對象為台南市申報牙醫師。本研究共分兩部份，第一部份以健保申報資料為主，從實施前後不同時期(民國86年7月至87年6月及89年7月至90年6月)的保險申報件數、點數及單項資料來評估實施前後牙醫師的醫療耗用情形；第二部份係採以問卷調查方式，探討牙醫師的滿意度及其醫療行為變化情形。

本研究結果發現，總額預算制度實施總申報件數減少，總申報點數增加，達到預期「以價制量」的效果，可能與牙醫師公會嚴格審查有關；實施前後，總申報數量增加最多的為「後牙複合樹脂充填-單面」及「後牙複合樹脂充填-雙面」；支付點數調高的11個項目中，有6項申報數量增加，在相同類別的服務項目中，牙醫師會傾向選擇給付點數較高、社會上流行的治療風格、風險較低且難度不高的項目；不同年齡別的醫師在總額預算制度實施前後的申報數量有不同變化；牙醫師對目前工作量或金錢報酬不滿意時會傾向追求增加醫療服務量。

The purpose of this research is to explore the impacts of global budget payment system on dental physician behavior. The research is focused on dentists in Tainan City, and the study is divided into two parts. First, in order to evaluate medical resource utilization from service volumes, service points claimed and more details, we uses the data between July 1997 to June 1998 and July 2000 to June 2001 on medical income from the National Health Insurance Research Database. Second, we explore the satisfaction of dentists to implementate global budget payment system and the impacts of physician behavior by a survey of dentists’ opinions from the questionnaire.

The research results indicated that total medical service volumes were significantly decreased and total service points claimed were significantly increased after implementation of global budget payment system. The anticipated benefits might have correlation of strictly peer reviewed by the medical professional groups. Besides, we found the maximum dental service volumes increased are “Posterior Teeth Composite Resin Restoration- Single surface” and “Posterior Teeth Composite Resin Restoration- Two surface”. We also found the change in service volume of six designated price-adjusted procedures increased after global budget payment system implementated compared with others unadjusted procedures ones. Dentists will prefer to add service volumes when the item with higher price-adjusted, popular treatment or lower risk and undifficulty. Furthermore, Dentists with different age made significant differences to service volumes and points claimed. Finally, dentists with less satisfied in service quantity or compensation might respond to the losses by increasing treatment volumes.
