音樂療法應用於減輕燒燙傷病患疼痛程度方面的有效成果評估,一般都是由醫護人員透過問卷調查、臨床觀察及相關醫療記錄等較為主觀的方式來評估,較缺乏客觀且量化之評估方式。紅外線熱影像具有非接觸性、客觀性、連續性的特點,因此本論文利用熱影像攝影的方式取得熱影像,再以影像處理技術來取得燒燙傷病患的體溫,利用體溫之變化及醫護人員相關記錄來做為音樂療法減輕疼痛之評估依據。
對燒燙傷病患所拍攝之熱影像,經由影像處理,擷取特定部分之溫度以進行音樂療法減輕疼痛之評估實驗。實驗結果則採用兩母體變異數未知且不等的t檢定進行二母體平均數差異之檢定。實驗結果顯示,利用熱影像技術來評估音樂療法下燒燙傷病患疼痛減輕程度,體溫呈現有效上升之趨勢,也就是音樂療法的確有助於燒燙傷病患放鬆身心,減輕疼痛。
Music therapy has been used to assist in pain and anxiety management of burn treatment for a long time. It was shown that the music therapy is one of effect methods for releasing the "feeling" of pain. This conclusion is conducted via the results of patient's questionnaire and subjective. The propose of this thesis is to try to verify the above result via an objective point.
Infrared thermography was captured from infrared camera and image processing technologies were used to preprocess the thermography. The temperature was extracted from patient's dorsal foot thermography. Twelve voluntary patients from the burn center of NCKU hospital are divided into two groups: one is given the listening of music and the other isn’t. The temperature are measured at every one minute by both of the thermography and ear's thermometer for comparison. To give the group with music therapy, 20 minutes of selected music is played for the patient. Two-tailed t-test was used as the tool for the test of the experimental result. Although the temperatures of two groups are both rising, the temperature gradient of two groups is different in statistics(P<0.05). The temperatures gradients of the group with music are 0.27℃and 0.6℃ in 10 and 20 minute, respectively. The other group is 0.14℃and 0.26℃, respectively. It means that patients of both group are under the situation of relaxing. From the result, it is shown that music therapy is an effective method to release the pain and anxiety management of burn treatment.
