  • 目的:近代許多研究探討巴金森氏病(巴病)病人的臉部情緒辨識能力及其與認知功能之關係,包括用於計劃和處理訊息的工作記憶和執行功能、用於辨識臉部特徵的視覺空間知覺能力,以及同時處理不同訊息來源的注意力功能等都曾被認為與臉部情緒辨識能力有關(Herrera, Cuetos, & Rodríguez-Ferreiro, 2011; J Pietschnig et al., 2016)。然而,至目前為止,不同領域之認知功能對臉部情緒辨識的影響仍然不清楚,本研究探究巴金森氏病合併輕度認知障礙病人不同的臉部情緒類別與其認知功能間之關係。
    方法:本研究之參與者來源分為兩類,分別為由原發性巴金森氏病病人組成的巴病組(共63人),以及經性別、年齡、教育程度與巴病組配對的健康對照組(共63人),巴病組的個案依動作障礙學會之輕度認知功能障礙診斷準則中的第二級標準(Litvan et al., 2012)分為兩組:巴病正常認知功能組(共21人)與巴病合併輕度認知障礙組(共42人)。所有參與者均須接受整體認知功能評估及人口學變項檢測,並以多模式情緒辨識應用程式來評估其情緒辨識能力,巴病組參與者亦使用全面性的神經心理學測驗來評估其各領域認知功能表現。
    結果:本研究發現並非所有巴病病人均有臉部情緒辨識障礙,僅巴病合併輕度認知障礙病人在臉部情緒辨識能力表現顯著較差(p = 0.001),其中更以「厭惡」(p < 0.001)情緒類別為主。巴病病人對負向與中性情緒類別的臉部情緒辨識表現與五個認知功能領域有所相關(包括注意力/工作記憶、執行功能、語言、記憶、視空間功能等),其中以負向情緒中「厭惡」的臉部情緒辨識表現與最多領域的認知功能相關,本研究更進一步發現,注意力功能部分中介了執行功能對「厭惡」臉部情緒辨識表現之預測(β = -0.297, p = 0.013)。
    討論:巴病合併輕度認知障礙病人難以辨識他人「厭惡」的臉部情緒,其集中注意力會影響認知靈活性(執行功能)對「厭惡」臉部情緒辨識的表現預測能力。基於本研究結果,建議未來可針對認知靈活性和集中注意力進行訓練以提高巴病合併輕度認知障礙病人對他人「厭惡」臉部情緒之辨識能力。

    The impairment of facial emotion recognition (FER) in Parkinson's disease (PD) patients has been investigated in many studies. But so far, the impact of cognitive dysfunction on FER and the FER in PD patients with mild cognitive impairment (PD-MCI) is still unclear. We aimed to explore the FER in PD-MCI and the cognitive function’s role in the FER. 126 participants were included in this study, including 63 idiopathic PD patients, and the healthy aging matched the PD patients by gender, age, and education level (n = 63). The FER of PD-MCI was worse than that of PD-NC and healthy aging (p = 0.001), especially in recognizing “disgust” facial emotions of others (p < 0.001). This deficit is correlated to the patient's attention, executive function, memory, and visual-spatial function. The attention partially mediated the impact of executive function on FER (β = -0.297, p = 0.013). We suggested that to improve the “disgust” FER ability, a specified intervention of cognitive training in cognitive flexibility and focus attention is needed.
