目的:肥胖目前在國內及國外都是一項重要的公共衛生議題,主要是肥胖的盛行率在各國都逐年的上升,而且因肥胖引起的慢性病,也逐漸的增加,如心血管疾病、高血壓、糖尿病、關節炎及癌症。因此如何降低體重避免上述併發症的提早發生,就顯得格外重要,過去傳統的減重方式是飲食控制、運動及行為治療,但是在減重過程中,常會面臨饑餓感而導致減重失敗;以往的研究,針炙運用在減重方面,可以降低食慾,達成減重的成效,我們想探討在傳統的減重方式上加入針炙,是否可以強化實質的體重降低的效果。
研究方法:以登報方式,收集身體質量指數大於25的肥胖者,年齡大於18歲以上,共50位,依據年齡、性別及身體質量指數配對,隨機分成二組,一組為針炙組,另一組為對照組,兩組在體重控制上皆施以飲食及運動計劃,唯一差別是針炙組,一星期有二次的體針及耳針,在進入計劃之前,完成個人體重、腰圍、血壓及各生化數值的測量,經過六週之後,再後測相同的指標,以了解針炙在兩組的影響。
結果:50位參加者,有45位完成全程計劃。在此計劃中我們發現不論是對照組或針炙組,其中的傳統減重方式,皆可以降低體重、身體質量指數、體脂肪率及各種生化數值,針炙組在體重及身體質量指數降低的幅度都大於對照組,不過在比較兩組間的不同時,針炙組並未達統計上顯著的差異,這說明針炙的介入並無強化傳統減重的效果。
結論:針炙在過去的研究中,雖說可以降低食慾,但是在傳統的減重方式介入之後,此項成效並沒有表現出,這說明過去針炙的研究只是單一的減重介入,藉由食慾的降低來減少熱量的攝入而達成減重成效,此種食慾的減少及增加胃飽足感,在傳統飲食控制中是可以藉由高纖維食物的攝食來加以改善,因此在減重方法中,並不需要加入針炙於傳統的減重方式。
Aims: Obesity is an important public issue in our country and the world. The prevalence of obesity has been increasing each year and the related chronic diseases, such as hypertension, diabetes, arthritis and cancer, are also increasing recently. To early prevent the co-morbidity of obesity by weight reduction is apparently important. Traditionally, the methods of weight control are diet control, exercise and behavior therapy. However, the hunger sensation during the weight control process often results in the failure of the program. Previous studies indicated that acupuncture could decrease the appetite and achieve the goal of weight control. The purpose of the study was to examine if acupuncture had the enhanced effect on traditional weight management method.
Methods: Fifty adults aged 18 years and older and BMI > 25 were recruited from the advisement of newspaper as our study sample. All participants were randomly divided into two groups, experimental (acupuncture) and control groups, according to their age, gender and BMI. Diet control and exercise programs were provided to both groups. Only acupuncture in auricle and body needle two times each week was applied to the experimental group. Both groups were required to complete six weeks weight reduction program and their personal weight, waist, blood pressure and biochemical profile were measured before and after the project.
Results: 45 adults completed the whole program. Results showed that traditional weight reduction method can decrease the weight, BMI, fat content and biochemical profile on both groups. The acupuncture group had greater magnitude of effect on weight loss and BMI decrease than the control group. However, no statistically significant difference was found between these two groups among the above measures which explained the acupuncture had no enhanced effect on weight reduction as compared with traditional weight control method.
Conclusion: Although previous studies found acupuncture had the effect of decreasing appetite to reach the goal of weight reduction, the study illustrated that this appetite decrease can be corrected by high fiber diet intake after the intervention of traditional weight reduction method. Therefore, no additional acupuncture is needed for weight advanced control.
