  • 目標:本研究之目標在探討青少年受到體重歧視(感知污名)到歧視內化(自我污名)及自尊影響心理健康(憂鬱、焦慮)路徑,並納入性別、真實體重及自覺體重分層分析。
    方法:本研究選取彰化縣某國中,於2017年5月到7月進行全校收案。問卷回收份數為465人,最後以457人為分析的樣本數。問卷內容包括個人基本資料、被歧視的經驗(感知污名)、自我污名、心理健康(焦慮、憂鬱、自尊)等變項,並依真實體重與自覺體重進行分組,最後將上述資料進行卡方、相關、變異數及中介路徑分析。本研究測試了兩個路徑模型: (1) Model 1假設自尊及自我污名皆扮演著感知污名與心理健康(憂鬱、焦慮)間的中介角色;(2) Model 2假設體重狀態會透過感知污名再透過自我污名影響心理健康。以上兩個路徑模型也加入性別作分析。
    結果:描述性統計發現自覺體重分組時,組間的感知污名、自我污名、焦慮、憂鬱及自尊有顯著差異。而中介路徑分析發現Model 1感知污名與心理健康(焦慮、憂鬱)相關,且自尊扮演中介因子,而自我污名則無;Model 2自覺體重很胖組和稍胖組皆比瘦及正常組更有機會透過感知污名影響憂鬱,但不會透過自我污名,以上兩個路徑模型都無法發現存在性別差異。
    結論:我們發現在青少年樣本中,感知污名與心理健康相關,但自我污名卻沒有。在分析感知污名與心理健康相關時,自覺體重比真實體重更能發現出組間差異。青少年的心理健康常是被忽視的一個議題,要避免學童遭受體重歧視,推行家長、教師、健康促進者提供孩子社會支持的相關政策可以透過提升自尊,降低體重歧視體重相關的污名與心理健康的關係。在臺灣目前較少有青少年研究體重相關污名化,期待本研究能提供政府做參考,在推行青少年減重政策的同時,也能考慮到對心理健康的影響。

    We examine the pathway of internalized stigma between weight bias (perceived stigma) and mental health in adolescents. We used a sample of 457 junior high school students for data analysis. Based on the mediation analysis, self-esteem is a more prominent mediator than internalized stigma in the pathway from perceived weight stigma to mental health in our sample. Adolescents who perceived themselves to be obese or overweight were more likely to exhibit the association between perceived stigma and depression, compared to those who perceived themselves as normal or underweight. Health practitioners, teachers and parents should pay extra attention to adolescents who perceived themselves as obese or overweight and provide means to improve their self-esteem.
