  • 背景:護理人員的急救能力對於能否成功搶救院內心跳停止病人是重要關鍵因素。盡早施予心跳停止病人高級心臟救命術,能提升病人恢復自發性循環的機率,改善病人的存活率及出院率。成人發生院內心跳停止的機率遠高於孩童,成人病房護理人員若未具備適當的急救能力,無法有效評估危象,啟動急救,將錯失急救良機;無法勝任急救任務,可能導致病人死亡;無法掌握急救情境,更容易引發醫療糾紛。目前研究尚未針對成人病房護理人員之急救能力及其相關因素進行探討。
    研究目的:探討成人病房護理人員之急救能力及其相關因素。
    研究方法:研究設計為橫斷性、相關性研究,採方便取樣,以自擬人口學變項問卷進行測量,了解成人病房護理人員之人口學現況;以護理人員自我評估執行急救能力量表進行測量,了解護理人員之急救能力。於南部某醫學中心成人病房收案,納入條件為一年內參與過急救或急救教育訓練之內科、外科,以及綜合科之成人病房臨床護理人員。排除條件為安寧病房護理人員,以及年資小於三個月護理人員。使用IBM SPSS Stastistics 23軟體進行資料分析,以描述性統計說明人口學變項及急救能力之現況;推論性統計則運用獨立樣本t檢定、單因子變異數分析,以及皮爾森相關係數了解人口學變項與急救能力之相關性,再運用多元線性迴歸分析探討人口學變項對於急救能力之影響因子。
    研究結果:研究對象以女性(n = 162;92.0%)、N2層級(n = 75;42.6%)、護理組長(n = 104;59.1%)、大學學歷(n =166;94.3%)、外科病房工作者(n = 70;39.8%)、有高級心臟救命術證照者(n = 153;86.4%)、最近一年內每月參與急救者居多(n = 46;26.1%);最近一次參與急救教育訓練之種類,則以參與團隊演練者最多(n = 89;50.6%)。年齡分佈在21歲至52歲之間,平均年齡28.5歲;平均護理工作年資6.2年;平均急診工作年資0.3年;平均加護病房工作年資0.8年;最近一年內參與之急救教育課堂數,以課室課程課堂數最多,平均2.9堂、團隊演練課程次之,平均2.7堂、以技術課程最少,平均2.1堂;最近一次參與急救教育時間間隔平均為0.2年;最近一次參與急救時間間隔平均為0.2年。本研究結果在護理人員自我評估執行急救能力量表之單題平均得分為4.0分,屬「大部分達到」。在各個次構面急救能力之得分,以「呼吸道及循環之維持」為最佳,單題平均得分為4.3分,以「異常心律之判讀及處置」最差,單題平均得分為3.7分。本研究之人口學變項與護理人員自我評估執行急救能力量表得分之多元迴歸模型,整體解釋變異量為30.7%,共有兩個因子具顯著相關性,分別為「N層級」(β= -.273 p = .035)、「最近一年內參與的急救教育-技術課程數」(β= .197 p = .031),代表N層級之護理人員自我評估執行急救能力量表得分顯著較低,最近一年內參與的急救教育-技術課程數愈多,護理人員自我評估執行急救能力量表得分顯著愈高。
    結論:本研究結果在護理人員自我評估執行急救能力屬「大部分達到」。以「呼吸道及循環之維持」次構面之急就能力為最佳;「異常心律之判讀及處置」次構面之急救能力為最差。N層級之護理人員急救能力顯著較差,最近一年內參與的急救教育-技術課程數愈多,護理人員急救能力愈好。

    Resuscitation competency among nurses is an important factor for the successful rescue of patients with cardiac arrest in the hospital. Early intervention of Advanced Cardiac Life Support to patients with cardiac arrest has been proven to increase the return of spontaneous circulation rates and improve patient survival rates and discharge rates. Adults are much more likely to have cardiac arrest in the hospital than children. Therefore, Resuscitation competency among nurses in adult wards is particularly important. If the nurses in the adult wards were not equipped with appropriate resuscitation competency, they would easily miss the first aid opportunity, and it might cause the death of the patients and cause medical disputes. However, Resuscitation competency and related factors among nurses in adult wards have not yet been explored. This study aim was exploring resuscitation competency and related factors among nurses in adult wards. A cross-sectional, correlational design was used. A convenience sample of 176 nurses were collected. The explained variance of self-assessment resuscitation competency among nurses from multiple linear regression model was 30.7%. There were two demographic variables as predictors, which were "N level" (β = - .273, p = .035), and "The numbers of resuscitation technical courses participated in the most recent year" (β = .197, p = .031). The result indicated a significantly lower scores of the self-assessment resuscitation competency among the N-level nurses, and the higher numbers of resuscitation technical courses that the nurses participated in the most recent year, the higher scores of the nurses’ self-assessment resuscitation competency.
