  • 背景與目的:頭頸癌高居台灣所有癌症第七位。較歐美地區盛行。頭頸癌常於肩負家庭重擔的青壯年時期發病。癌症本身與醫療伴隨而來的骨骼肌肉系統、外觀、飲食、吞嚥等功能損傷往往使患者工作能力減損或延宕復工,經濟陷入困頓。本研究主要目的有二:(1)探討已復工和未復工患者之功能表現與復工狀況差異,(2)探討功能表現對復工狀況之影響。
    方法:本研究針對完成癌症治療5年內之頭頸癌患者,進行以醫院病患為基礎的研究。建置「頭頸癌病友復工調查表」問卷與功能評估。共招募69位患者,其中38位為已復工者,31位為未復工者。本研究分析比較已復工者與未復工兩者之差異,及與功能表現與復工狀況之關聯。
    結果:已復工者與未復工者差異比較,在功能表現上:未復工者於頸部旋轉與張口度損失較大。且活動程度、休閒娛樂、吞嚥、咀嚼、講話功能與自評功能表現也較差。在復工狀況上:未復工者停工時間較長,且較高比例患者於罹癌前主要從事藍領工作。整體頭頸癌患者復工與否則和患者自評功能表現、休閒娛樂、講話功能、張口度有重要關聯。
    結論:頭頸癌患者容易因功能表現損傷影響復工機率。未復工患者在功能表現上仍較已復工患者為差,且停工時間更長。此結果可提供職能治療、復健與職業健康專業發展復工指引參考方向。

    Head and Neck cancer (HNC) is more prevalent in Taiwan than western countries because of widespread habits of betel quid chewing, tobacco use, and alcohol consumption. These patients with HNC are usually the main source of household income and need to struggle with functional impairment in return to work (RTW) after cancer diagnosis and treatment. However, the RTW model with professional support is limited and lack of investigation in Taiwan and other advanced countries. This was hospital based study aimed to (1) compare difference between patient successfully returned to work with those did not return to work, (2) investigate the correlation with RTW and functional performance in patients with HNC. Sixty-night paients completed the curative treatment of cancer were recuited. They were invited to ansewer a self-designed questionnaire and functional evaluation—“The Functional Performance and Return to Work in Patients with HNC.” The results showed that patients did not return to work had more loss of cervical rotation and mouth opening than those successfully returned to work. They also perceived more limitation in activity, recreation, swallowing, chewing, and speech. In RTW, they moslty did blue-collar jobs before. It took more time for them to RTW. Moreover, RTW had significant correlation with self-perceived functional performance, recreation, speech, and mouth opening in patients with HNC. This study provided valuable information for occupational therapy and other rehabilitation disciplines to construct a guideline for functional training and RTW intervention for patients with HNC.
