背景:老年感染者由於面臨老化以及疾病病程,有著更多的生理及心理上的症狀,社會支持可以有效的增加感染者的心理調適以及服藥遵從性延緩疾病進程,進一步改善生活品質。
目的:本研究旨在探討老年感染者與年輕感染者在社會支持、生活品質與死亡率的不同以及其相互關係,並進一步預測影響生活品質的重要因子。
方法:本研究為回溯性、橫斷性研究,採次級資料庫分析,以南部某醫學中心愛滋病個案管理資料庫中共246人之資料進行分析,經個案管理師以結構式問卷包括:自覺社會支持多面向量表、世界衛生組織生活品質量表簡明版(WHOQOL-HIV BRIEF)及病歷回顧方式資料蒐集。以線性迴歸分析CD4數目、社會支持以及共病指數等對生活品質之預測能力。
結果:老年感染者的社會支持低於年輕感染者(25.8±7.61 vs 31.83±9.118, p=.04),尤其在朋友面向的支持顯著較少(6.70±2.71 vs 10.35±3.78, p=.003)。社會支持與生活品質呈現中度相關。而預測感染者生活品質因素主要為社會支持(F=8.252, p<.001)。
結論:增加感染者之社會支持可作為增進HIV感染者之生活品質介入措施之參考。
ABSTRACT
Background:The HIV older adults had more physical and psychological symptoms because of being confronted with the aging and disease progression. Social support can effectively enhance the coping strategy and adherence of people living with HIV, delay their disease progression, and furthermore improve their quality of life.
Objective:To discuss the differences and correlations in social support, quality of life and mortality rate between older and younger adults in HIV, and furthermore to predict factors on quality of life.
Methods:Secondary database analysis was conducted in the study. Total 246 patients in the HIV Case Management Database of a university hospital in southern Taiwan were included in this analysis. Data was collected through chart review and structured questionnaires including: MSPSS, WHOQOL-BREF HIV. Linear regression was used to analyze the predicting ability of CD4 cell counts, social support and Charlson Comobidity Index on the quality of life.
Results:Social support of HIV older adults was lower than younger ones ( 25.8±7.61 vs 31.83±9.118, p=.04 ), especially in the friend domain ( 6.70±2.71 vs 10.35±3.78, p=.003 ). Social support moderately correlated with the quality of life of people living with HIV, and was the main factor to predict their quality of life (F=8.252, p<.001).
Conclusion:Increasing social support provides reference for further intervention to improve the quality of life of people living with HIV.
