研究背景:輕度認知功能障礙(Mild Cognitive Impairment, MCI)常發生於老年人,其發生率及盛行率因人口老化而逐年增加,而許多輕度認知功能障礙的患者會進展至阿茲海默氏症可能會影響生活品質。目前國內仍缺乏比較一般認知正常與輕度認知功能障礙的社區老年人之生活品質的研究。故本研究將比較正常與輕度認知功能障礙的社區老人之生活品質,並探究兩類老人各生活品質範疇之差異性。此外,亦探討兩組個案之各認知功能範疇之狀況。
研究方法:本研究屬橫斷性研究設計,採方便取樣方式,配合大台南市社區健康篩檢活動,針對60歲及以上自願民眾,以聖路易大學心智狀態測驗(SLUMS)及世界衛生組織生活品質問卷-臺灣簡明版(WHOQOL-BREF)等結構式問卷收集參與個案資料。以SPSS 17.0統計軟體進行分析,針對基本的社會人口學變項、SLUMS量表總分、生活品質各範疇的得分採描述性統計;進一步使用獨立樣本t檢定比較組間的差異,檢定不同認知功能範疇個案期生活品質是否有差異。
研究結果:
本研究發現兩組個案在動物命名、數字序列、記憶力、執行功能與回覆能力等五個認知功能範疇有顯著差異,但認知功能正常者與輕度認知功能障礙者在整體生活品質及生理、心理、社會關係及環境範疇的比較沒有顯著差異。
結論:本研究結果顯示兩組對象在部分認知功能範疇上有顯著差異,但是早期認知功能缺損不會對社區老人生活品質造成影響,而藉由社區認知功能篩檢,讓社區輕度認知功能障礙者能夠及早接受認知功能相關健康保健知識,使其維持生理健康、心理狀態、獨立程度、社會關係、個人信念及環境六大層面之平衡,達生理、心理及社會之安適狀態。
Quality of life was usually affected by cognitive defect, the purpose of this study was to compare the difference of quality of life between cognitive intact and impaired elders and to explore differences among dimensions of quality of life. In addition, domains of cognition between two groups were compared. This study was a cross-sectional design. Convenience sampling was used to recruit 60 years old or older community-dwelling adult volunteers through routine health screening in Tainan. Instruments include SLUMS was used to assess cognitive status of subjects and WHOQOL-BREF Taiwan version was used to assess quality of life of the subjects. independent t-test was used to test the difference of cognitive domains and quality of life between groups. Results showed differences in some cognitive domains However there was no significant difference in quality of life between normal and MCI patients, and among domains in physical, social relationship, environmental QOL. Findings of this study showed differences in some cognitive domains between two groups, however mind cognitive impairment did not affect quality of life of community dwelling elders.
