前言:家屬照顧者在社區失智照顧上扮演重要性角色。過去研究關注於照顧壓力產生的過程,以及如何改善照顧者的負向照顧經驗,較少以提升照顧者的正向照顧經驗為策略,以達到促進安適感的臨床介入。另一方面照顧工作的實際執行與照顧的意義會受到社會文化信念影響,了解家屬照顧者在這樣的文化思維裡的行動和照顧經驗有助於臨床介入。本研究目的是探索失智症成年女兒照顧者在日常生活中所從事照顧活動,以便認識正向照顧經驗的內涵和特質,及其發生的情境脈絡。
研究方法:本研究採用目的性選樣,從家屬支持團體和失智症相關協會招募六位成年女兒照顧者。使用半結構式深度訪談、職能日誌、以及參與式觀察收集資料。訪談全程錄音轉成逐字稿,所有研究資料以紮根理論進行分析。
研究結果:失智症成年女兒照顧者所獲得的正向照顧經驗,來自照顧者透過日常職能與「失智者」和「所處環境」進行互動,照顧者體驗或評價其日常職能後,獲得正向結果的過程。照顧者所獲得的結果包含聯繫親情、照顧成就感、生活樂趣、安心自在、個人成長、善盡孝道。
結論:本研究發現正向照顧經驗與安適感有許多重疊的特質,個人解讀事件的方式和社會文化情境影響照顧者的正向照顧經驗。此外照顧者在調整照顧模式的過程,發展出新的孝道實踐方式來符合照顧情境所需,以兩害相權取其輕的原則取代事事順從父母的傳統思維。然而當前的社會仍對女兒照顧者從事照顧原生父母採取保守的態度,因此未來社會應對照顧原生父母的女性給予友善的支持。
Introduction: Family caregivers play a vital role in community-based dementia care. In the past, most studies mainly explored the sources of caregiving stress and used a stress-oriented approach to ameliorate the negative aspects of caregiving experience. Rarely has the research explored the possibility of using strategies to enhance positive aspects of caregiving experience, and thus promoting the well-being of caregivers. The meanings and practice of caregiv¬ing are also influenced by sociocultural beliefs. Understanding the positive aspects of caregiving experience situated the unique socio-cultural contexts in Taiwan can contribute to the developmental of effective clinical interventions. This study explores the caregiving activities that occurs in the daily routines of adult-daughter caregivers for parents with dementia. The goal is to understand the positive aspects of their caregiving experience and its cultural contexts.
Methods: A purposive sample of six adult-daughter caregivers were recruited from support groups and dementia associations in Taiwan. In-depth semi-structured interviews with occupational dairies and participant observations were used to collect data. The interviews were audiotaped and transcribed verbatim. All the data were analyzed with grounded theory.
Results: Adult-daughter caregivers experienced and interpreted the daily occupation as positive through the results from interaction with their parents with dementia and the immediate context. The characteristics of the experience include a sense of familial relatedness, accomplishment of being a good caregiver, enjoyment of life, a sense of being at peace with oneself, personal growth, and fulfillment of filial piety.
Conclusion: The study found the characteristics of positive caregiving is similar to those of well-being. The meanings caregivers ascribed to their daily encounters with their parents and certain sociocultural beliefs influenced their interpretation of the experience. Caregivers also redefined the practice of filial piety to meet the demands of caring for parents with dementia. As opposed to the traditional filial piety that dictate absolute obedience, daughter caregivers often faced the dilemma of having to choose the lesser of two evils in making caregiving decisions. Social norms in Taiwan are not always supportive of daughters who care for their own parents.
