  • 本文以內生成長模型為架構,探討預防性保健支出及診療性醫療支出的分配對生活品質調整後存活人年的影響。研究發現:政府投入預防性保健實施,使部分診療性費用降低,產生醫療節約效果,進而達到加速經濟成長及提升社會福利。然而,預防性保健支出對經濟成長及社會福利的影響為一非線性關係;當健康支出增加超過最適值時,會產生排擠其它公共支出導引資本累積下降進而妨礙經濟成長。並且,經濟成長與社會福利極大值下的健康支出二者間有所差異。再而,利用生活品質調整後存活人年(Quality-Adjusted-Life Expectancy, QALE)刻畫健康計量單位效用,得到預防性醫療支出持續增加至最適值對於增進人民健康壽命具有改善效果。當預防性保健支出從現行占GDP約0.27%提高至最適值1.19%時,會達到經濟極大值並且增加5.03QALY;而當預防性支出增加至2.03%時,達到社會福利極大化且增加5.56QALY。

    Based on endogenous economic growth theory, this research investigates the effects of allocating preventive and treatment health expenditure on economic growth and social welfare. Prevention devotion is expected to reduce future curative health expenditure, save part of healthcare expenditure, and improve the population’s health stock. Theoretical analysis indicates that the relationship between preventive health expenditure and economic growth or social welfare is non-linear. Economic growth and social welfare both increase when the ratio of preventive health expenditure to GDP below the optimum, otherwise, economic growth becomes slower and social welfare deteriorates. The welfare-maximizing share of spending on prevention health care exceeds that for growth maximization. Applying Taiwan’s experiences, this research further conducts calibration to verify theoretical findings. Using the method of quality-adjusted life expectancy (QALE), this research estimates the health variations. When the ratio of preventive health expenditure to GDP increases from current devotion, 0.27 per cent to the optimal, 1.19 per cent, will attain the growth-maximization and increases by 5.03 quality-adjusted life years(QALY); when preventive healthcare expenditure increase to 2.03 per cent, will get the welfare-maximization and increases by 5.56 QALY.
