  • 背景:
    復元導向服務 (recovery-oriented services)已是當前世界心理衛生領域的主流,此觀點重視精神障礙者對個人及疾病的主觀感受,認為即使症狀存在,精障者仍然可以過著有希望的生活。然而目前台灣復元導向服務尚在起步階段,許多精障者對復元概念亦不熟悉,目前雖有研究使用疾病管理與復元方案進行介入,然而方案內容較侷限為疾病管理觀點,且較少了解精障者對自己的個人看法及感受,因此本研究驗證一個符合台灣文化且協助精障者朝向復元之方案成效,了解該方案對於精障者整體復元程度以及重要復元指標的影響。

    方法:
    本研究為隨機分派介入研究,研究對象為符合DSM-5診斷之嚴重精神疾病障礙者。收案方式採方便取樣,由研究人員於南區兩家社區復健中心招募參與者,並將其隨機分派至介入組及控制組。介入團體分為兩階段共20堂課程,每週一次,每次1.5小時。第一階段簡介復元概念及探索個人優勢,第二階段協助成員設定目標與計畫。控制組成員將維持常規治療,並可獲得一本疾病衛教書籍供其自行閱讀。本研究進行前後測資料蒐集,收案時進行前測,由成員填寫復元階段量表、自尊量表、社會支持問卷、希望量表、簡式精神症狀評量表;介入組於課程結束後再填寫前述量表進行後測,控制組則配合介入組評量時間進行評量。本研究資料採治療意向分析法,使用單因子共變數分析,將前測作為共變數以檢定復元及各項復元指標之組間差異。

    結果:
    本研究共召募54位受試者,介入組與控制組各有27名成員,兩組人口學資料及前測結果皆無顯著差異。研究結果顯示與控制組相比,介入組在復元階段量表 (p = 0.032)、希望量表 (p = 0.011)與社會支持系統問卷 (p = 0.020)皆達顯著改善。精神症狀評量雖兩組總分未有顯著差異,然介入組部分項目在組間達顯著改善,如身體化症狀、人際敏感、焦慮、敵意、精神病性及附加症狀等。兩組在自尊量表分數則未達顯著差異。

    結論:
    精神障礙者可透過此復元方案提升個人整體復元程度,且能使精神障礙者認識個人優勢與資源,對於個人未來及目標有較好的希望感及動力,並提升他們對個人社會支持系統的正向感受。未來,可將此方案應用於臨床服務,增加專業人員及精障者對復元的認識,以落實復元導向的精神復健服務。此外,建議未來可持續進行研究以增加收案量,建立方案成效,確保方案成效可類推至所有族群。

    Recovery oriented approach has been promoted in mental health field around the world. This approach emphasizes that people with mental illness can live a hopeful life even though symptoms still exist. However, few recovery programs for people with mental illness are available in Taiwan. The purpose of the study was to evaluate the effectiveness of a recovery program for people with mental illness in Taiwan.

    A randomized controlled trial was conducted at two community mental health centers in the south of Taiwan . Fifty four people with mental illness were randomly assigned to a recovery program or treatment as usual (TAU). Participants of the intervention group gathered for a 1.5-hr session once a week for 20 weeks. Assessments were conducted at baseline and posttreatment . Outcome measures included Stages of Recovery Scale (SRS), Rosenberg Self Esteem Scale (RSES), The Hope Scale (THS), Medical Outcomes Study Social Support Survey (MOS-SS), and Brief Symptom Rating Scale-50 (BSRS-50).

    Participants of the intervention group demonstrated significantly improvement in Stages of Recovery Scale , The Hope Scale, and Medical Outcomes Study Social Support Survey than the control group. T here were no significant differences between the t wo groups on Rosenberg Self-Esteem Scale and Brief Symptom Rating Scale-50.

    The recovery program could help people with mental illness to aware their strengths and resources as well as improve their social support and sense of hope. It is beneficial for recovery of people with mental illness and can be applied widely in Taiwan.
