  • 下背痛是護理人員普遍常見的肌肉骨骼健康問題,於醫療健康照護者中高於其它職業,因而請病假、曠職,甚至導致離職及失能。下背痛會反覆發作影響日常生活功能以及工作上的表現,為一重要問題且不容忽視。本研究目的預探討影響下背痛護理人員不同功能性表徵之相關因子。
    本研究為橫斷式調查研究設計,以自填式問卷調查方式收集資料,針對南部某醫學中心臨床護理人員進行調查,樣本數共納入480人。資料收集工具為基本人口學資料以及奧斯福下背痛量表之問卷調查,進行臨床護理師下背痛相關資料收集。統計方法以描述性統計、卡方、t檢定與邏輯斯迴歸分析進行資料統計分析。
    研究結果顯示下背痛盛行率74.6%,以輕度為居多(佔86.9%),且有71.5%伴隨頸部或肩膀疼痛,有高達77.9%下背痛護理人員曾因下背痛影響到臨床照護工作。心理社會因子中的負面情緒(p = .027, B = 1.625, EXP(B) = 5.077, 95% CI = 1.203~21.423),經常總是有負面情緒的護理師得到中重度下背痛的機會,是從未偶爾有時有負面情緒的護理師的5.077倍;職業因子中的每週工時(p = .036, B = 1.094, EXP(B) = 2.987, 95% CI = 1.074~8.308),每周工時大於50小時的護理人員,得到中重度下背痛的機會是每周工時小於50小時護理師的2.987倍。
    護理人員下背痛盛行率高,常伴隨其它部位的疼痛,下背痛程度主要以輕度失能為主,負面情緒與每週工時大於50小時與中重度失能下背痛具有相關性。定期為臨床護理人員進行下背痛檢測與評估嚴重程度,針對中重度失能下背痛護理人員給予主動關懷與密切追蹤,負面情緒的改善以及每周工時的調整考量,建議都需要納入員工健康促進計畫中,降低護理人員下背痛或嚴重失能情形,進而提升臨床照護工作的品質,降低護理人員離職或轉換工作想法,留住臨床職業之護理人力。

    Low back pain is a common health issue among nursing staffs, which may affect clinical practice and result in sick leaves, resignation or even disability. This study aims on exploring the related factors of different functional characters among nurses with low back pain. This is a cross-sectional study, using questionnaire within a Southern Taiwanese medical center, and the data were analyzed using the SPSS-26 system. Prevalence rate of low back pain was calculated as 74.6%, with co-morbid of shoulder or neck pain accounting for 71.5% of nursing staff. The majority of 86.9% resulted in mild disability, and 77.9% of nursing staff suffered from low back pain related effect on clinical practice. Nursing staffs that suffered from negative mood most of the times had an odds ratio of 5.077(p = .027, B = 1.625, EXP(B) = 5.077, 95% CI = 1.203~21.423) of suffering low back pain as compared with those who seldom suffer from negative mood. As for the working time, those who work over 50 hours suffer more from moderate to severe low back pain as compared with those who does not(p = .036, B = 1.094, EXP(B) = 2.987, 95% CI = 1.074~8.308). In conclusion, the prevalence rate of low back pain among nursing staff is high, and mostly result in mild disability; those with negative mood most of the times and weekly working exceeding 50 hours have higher risk of moderate to severe low back pain. Thus, the improvement of negative mood, as well as working time may be considered to be added to the health promotion program of employees.
