  • 本研究將電腦輔助手術科技應用在脊椎椎足鑽孔需高精確度定位的手術,可藉由術前的電腦輔助手術規劃與術中導引提高手術的安全性。客製化之鑽孔導引件可輔助醫師安全植入骨釘,在手術上實質協助醫師確定空間中骨釘植入方位,正確地導引螺絲釘鑽入狹小的椎足通道,降低因誤傷脊椎核與神經根引起的風險。
    本研究是以斷層掃描影像出發,提供醫師在電腦上做術前的鑽孔手術規劃,利用數理幾何方法配合醫科解剖學理,融入專業醫師臨床經驗,進行電腦輔助脊椎椎足鑽孔導引件的開發,在臨床手術中導引醫師空間中骨釘植入方位,客製化導引件可緊密包覆椎體表面幾何並穩固置放於椎體上,使鑽孔導引件準確定位,最後以快速原型機加工產生術中可用之手術定位輔件。

    In this study, we develop a computer-aided procedure to produce personalized drill guides that are designed to enhance the safety of the insertion of pedicle screws in the spinal region. Appliances such as transpedicular screws and rods require fixation through narrow channels called pedicles. Trajectories through these narrow channels of bone have optimal placement and limited tolerances. Complications can arise when the screws accidentally enter nerve roots or spinal spaces and transect the spinal cord.
    Based on a patient’s CT data it is possible to design personalized drill guides in a self-developed computer-aided design environment. These guides are designed to fit on the surface of the vertebrae in a unique position so as to indicate the pre-planned drill path and drilling length into the pedicle. Finally, the drill guides were manufactured by rapid prototyping.
