為了降低非傳染性疾病(NCDs)的數量,印尼政府啟動了「健康社區運動」(Gerakan Masyarakat Hidup Sehat,GERMAS),旨在促進健康生活行為。作為一個跨部門的計畫,GERMAS在實施過程中面臨許多挑戰。本研究以印尼特別省份日惹為例,探討 GERMAS 中跨部門合作的動態行為。參與日惹 GERMAS 的組織包括省政府、縣市政府、社區組織、非政府組織(NGO)、大學,以及印尼國家警察和印尼國家軍隊。日惹的 GERMAS 跨域合作由省長命令規範,這是一項對所有相關利益關係者具有約束力的規定。儘管 GERMAS 被設計為一個跨部門協作的計畫,但其實施仍主要被視為健康部門的政策,而非真正的跨部門政策,因此承諾和政治領導在確保每個部門積極參與並有效推行該計畫方面扮演著重要角色。此外,如果這個計畫可以基於參與部門的倡議來確定,部門間的計畫協調將會更加容易。感知差距、部門自我中心及財政限制是GERMAS合作中的主要挑戰,因為每個部門都有自己的優先事項。為了優化 GERMAS 的實施,利害關係人們需要統一其感知,在規劃和實施過程中協同合作,確保監測和評估機制到位以改進計畫,並確保資源的可用性以執行計畫。
As an effort to reduce the number of NCDs, the Indonesian government has launched the Healthy Community Movement, or GERMAS (Gerakan Masyarakat Hidup Sehat), aimed at promoting healthy living behaviour. GERMAS, as a cross-sectoral program, faces many challenges in its implementation. This research uses the Yogyakarta Special Province to capture the dynamics of cross-sectoral collaboration within GERMAS. Various actors involved in GERMAS in Yogyakarta include the provincial government, regency/city governments, community organizations, NGOs, universities, as well as the Indonesian National Police and the Indonesian National Army. The collaboration of GERMAS in Yogyakarta is regulated by a Governor Regulation that is binding on all stakeholders involved. Although GERMAS has been designed as a cross-sectoral collaborative program, its implementation is still viewed primarily as a health sector policy rather than a cross-sectoral policy. Therefore, commitment and political leadership play important roles in ensuring that each sector actively participates and adopts the program effectively. Additionally, program synchronization between sectors will be easier if the programs are determined based on the initiative of the involved sectors. Perception gaps, sectoral ego, and financial limitations are examples of the main challenges in collaboration, as each sector has its own priorities. To optimize the implementation of GERMAS, it is important for the involved stakeholders to align their perceptions, synergize in the planning and implementation process, ensure the monitoring and evaluation mechanisms are in place for program improvement, and ensure the availability of resources for program execution.
