根據各國經驗,無論再怎麼謹慎,醫療人員都可能發生錯誤;與其追究「誰」犯了錯並加以處罰,不如研究「如何」避免同樣的錯誤發生。為此本研究設計了一套針對醫療環境中協助醫療人員維護病人就醫安全的機制。本研究假設在設定的環境中,所有需要被注意的醫療流程相關人員、設備以及最重要的病患,皆可在確保隱私的前提下以電子標籤加以標記。我們發展出「以多限制條件為基礎之授權啟動」預防機制,透過安裝自行開發的行動醫療裝置代理程式(Medical System Agent)於可被管控或用於流程管控的醫療相關設備,以達到維護就醫安全之目的,並透過應用場景以驗證架構的可行性。本研究提出的預防機制是在預設的所有條件俱足時,例如包含正確的病患身份、合格的醫療相關人員、行動醫療系統(設備)…等,透過即時定位的機制加以判定是否已進入(或離開)規定允許(或拒絕)的使用空間範圍內,以及完成防護措施等的條件下,最後附加時間戳記(Timestamp)以確認所有條件均滿足於同一時間及同一地點的要求後,方可即時取得授權以啟動或操作行動醫療設備並產生紀錄。目前的原型系統選用在無線網路的基礎架構上之Wi-Fi標籤實測結果顯示,定位誤差可在2公尺內,定位時間週期縮短至每3~7秒/次,可在15~20秒達到穩定狀態,證實本研究所提出之雛型架構可適用於醫療環境。
According to international experiences, no matter how carefully each medical personnel does one’s the job, it is not possible to completely prevent medical staff from causing errors. Rather than looking for who is responsible for the error, it is better to find a way to prevent the error. Therefore, we design a RFID-based mechanism to help medical staff to improve patient’s medical safety. It is assumed in this system all related medical personnel, medical systems and patients are equipped with RFID tags under well-protected privacy. This proposed mechanism, named "multi-constraint-based authorization mechanism" , relies on a Medical System Agent which is installed on a medical system to control/enable the flow of the medical system to improve patient safety. The agent checks whether all predefined constraints, such as that the presence of patient identity, medical staff and medical systems, and they are within a defined region, are fulfilled. The infrastructure of the proposed system is a Wi-Fi wireless network, and a Wi-Fi based locating software for Wi-Fi tags. We develop scenarios to demonstrate the feasibility of the proposed system. In addition, a timestamp is also required to ensure that all conditions are met within an acceptable time interval. Our testing results show the locating deviation is within 2 meters and locating period needs 15 to 20 seconds to reach stabilization.
