煙火的釋放在世界各地廣泛地被用於慶祝活動中,台灣每年的元宵節期間鹽水鎮的武廟會施放蜂炮。本研究利用病例交叉研究評估釋放鹽水蜂炮產生之微粒污染物和循環系統、呼吸系統疾病門診的關係。
門診內容資訊來自全民健康保險資料庫,而環境暴露資料的來源是2004年、2005年、和2007年的元宵節期間派移動性空氣品質監測車前前往鹽水連續監測空氣污染濃度,以瞭解施放煙火短時間內釋放的污染物濃度。
結果發現,施放蜂炮當天晚上,各污染物濃度會急速地增加,蜂炮日的污染物濃度在01 lag、02lag、03 lag時也高於對照日的濃度,PM¬10和循環系統(OR:1.41, 95% CI: 1.37 -1.45, 每增加10 μg/m3)、高血壓(OR:1.47, 95% CI: 1.43 -1.52, 每增加10 μg/m3)、缺血性心臟病(OR:1.25, 95% CI: 1.14 -1.37, 每增加10 μg/m3)、呼吸系統疾病(OR:1.42, 95% CI: 1.40 -1.44, 每增加10 μg/m3) 、上呼吸道感染(OR:1.42 95% CI: 1.40 -1.43, 每增加10 μg/m3) 、氣喘(OR:1.42, 95% CI: 1.31 -1.55, 每增加10 μg/m3) 、COPD(OR:1.54, 95% CI: 1.50 -1.58, 每增加10 μg/m3) 、肺炎及細支氣管炎的(OR:1.54, 95% CI: 1.50 -1.58, 每增加10 μg/m3)。
因此,蜂炮產生的污染物增加循環系統、呼吸系統疾病的門診就診危險性,所以觀看煙火時仍應該重視對於自身的防護,減少身體不適的風險。
Fireworks are used worldwide as a part of the celebrations. In Taiwan, the Guan Yu Temple is known because of the Yanshuei Fireworks during the Lantern Festival every year. This study applied case-crossover study design to investigate the association between exposure to particulate air pollutants emitted from Yanshuei Firecrackers and the outpatient visits for circulation and respiratory disease. The information on outpatient visits were abstracted National Health Insurance database. Environment exposure data were from the Environmental Protection Agency (EPA). Air pollutants from the festival were carried out by EPA in 2003, 2004, and 2007.
We found out that the concentration of air pollutants were high on the evening of Lantern Festival and at 01 lag、02lag、03 lag. PM10 was significant associated with the clinical visits of circulation disease(OR:1.41, 95% CI: 1.37 -1.45, per 10 μg/m3 increase)、hypertension(OR:1.47, 95% CI: 1.43 -1.52, per 10 μg/m3 increase)、ischemic Heart Disease(OR:1.25, 95% CI: 1.14 -1.37, per10 μg/m3 increase)、respiratory System(OR:1.42, 95% CI: 1.40 -1.44, per 10 μg/m3 increase) 、upper respiratory infection(OR:1.42 95% CI: 1.40 -1.43, per 10 μg/m3 increase) 、asthma(OR:1.42, 95% CI: 1.31 -1.55, per 10 μg/m3 increase) 、COPD(OR:1.54, 95% CI: 1.50 -1.58, per 10 μg/m3 increase) and pneumonia and bronchiolitis(OR:1.54, 95% CI: 1.50 -1.58, per 10 μg/m3 increase).
Consequently, air pollution emitted from Yanshuei Firecrackers Ceremony increases the risk of clinic visits. We still need to take certain precautions in order to prevent health effect from the firecrackers.
