檢驗在現代醫療中至關重要,臨床實驗室為疾病診斷、預防與控制提供關鍵支持。隨著人口高齡化,台灣醫療院所的就診率與檢驗量持續上升,人為失誤也成為一大挑戰。為了因應檢驗量的增加並降低錯誤,引進自動化技術為可能的解決方案。然而,醫學檢驗自動化雖能提升檢驗效率與精準性,卻也改變了醫檢師的工作型態,帶來新的壓力。無論是檢驗自動化對醫療品質的影響,或是醫檢師在自動化環境下的職能調適,均需要被更深入探討。目前自動化相關研究多聚焦於檢驗效益,對醫檢師的影響探討有限。本研究基於成大醫院病理部血液組、血庫組、生化組、及檢體收受中心的資料,收集並分析影響檢驗效率與品質的相關效益指標,並比較自動化實施前後的差異。效益指標包括總檢驗量、人均檢驗量、TAT(turnaround time)、時效達成率、月時效未達閾值的次數及重檢率。醫檢師暴觸檢體的風險以流程圖及檢體接觸次數表示。同時透過問卷了解影響醫檢師工作滿意度的主要因素及自動化的影響。研究對象為成大醫院病理部的醫檢師,自動化問卷調查的對象為血庫組、血液組、微生物組及檢體收受中心的醫檢師。本研究結果顯示,以血液組自動化的檢驗效率而言,在引進當年的平均時效達成率下降5.3%。血庫的自動備血則優化了急件備血的時效,引進隔年與引進前相比,急件平均時效達成率上升了2.97%,月時效未達閾值率從7次下降到0次,且重檢率呈現逐月下降的趨勢。一般件備血的時效在自動化引進當年雖表現不如預期,但之後則漸有優化。生化組引進供收受中心使用的送檢體機器人對緊急生化及緊急血液的時效有顯著優化。在流程整合後,血液及血庫組的醫檢師接觸檢體次數顯著減少,檢體暴觸風險降低。問卷結果(64份回收,滿意度平均3.53分)顯示,與工作滿意度最相關的因素依次包括:報酬(τ = 0.68)、管理單位重視意見(τ = 0.43)、工作有挑戰性(τ = 0.35)及社會認同(τ = 0.38)。參與自動化檢驗工作的醫檢師普遍認為:自動化提升了工作滿意度(滿意度平均4.04)與效率,並減少了工作時間與壓力。但對某些檢驗項目的效益則仍有進步空間,例如:抗體篩檢和自動化血球分型計數。整體而言,自動化在提升檢驗質量與滿意度方面具有正向影響,但仍無法完全取代人力,需進一步優化以滿足實務需求。
Laboratory testing is crucial for diagnosing, preventing, and controlling diseases, playing a key role in public health. In Taiwan, increasing medical visits have put pressure on medical laboratories. Despite their importance, diagnostic errors, particularly human errors, remain a challenge. Automation has been introduced to reduce errors, improve efficiency, and lighten the workload of medical laboratory scientists. This study examines the impact of automation on laboratory performance at National Cheng Kung University Hospital, focusing on the section of hematology, blood bank, microbiology, clinical biochemistry, and specimen reception. It also explores how automation affects job satisfaction and stress through questionnaires. By comparing pre- and post-automation results, we found that automation had limited impact on the efficiency in the hematology section, but it significantly improved the timeliness for urgent blood preparation in the blood bank and for urgent biochemical and hematological tests. The factors most related to job satisfaction, in order, are: compensation (τ = 0.68), management's attention to feedback (τ = 0.43), job challenge (τ = 0.35), and social recognition (τ = 0.38). Medical laboratory scientists involved in automated testing generally believe that automation has improved job satisfaction and efficiency. Overall, automation has a positive impact on improving testing quality and satisfaction, but it cannot fully replace human labor and requires further optimization to meet clinical needs.
