Background: Caregivers may experience multiple roles during caregiving of patients with advanced heart failure (AHF) due to patients’ dependency is increasing. The caregiving tasks may create supportive care needs of the caregivers since they are laypersons. Previous studies reported that lower satisfaction with care has been shown to be associated with higher needs in chronic disease population. An early assessment of satisfaction with care, then addressing their needs is important to ensure the quality of care of the caregivers and patients with AHF.
Purpose: This cross-sectional study examined the relationship between satisfaction with care and supportive care needs among caregivers of patients with AHF in Indonesia.
Methods: A cross-sectional study with a convenience sample of 157 caregivers was recruited from general ward, cardiac ward, and cardiac care unit (CCU) in two public referral hospitals in Central Java and West Java, Indonesia. Satisfaction with care was measured using the revised Family Satisfaction with Advanced Cancer Care-6 (FAMCARE-6) questionnaire and supportive care needs of caregivers was measured with Supportive Care Needs Survey for Partners & Caregivers (SCNS-P&C) questionnaire. Descriptive statistics, bivariate analyses (independent t-tests, one-way ANOVA, and Pearson correlation), and hierarchical regression analysis were employed to analyse the data.
Results: A total of 156 caregivers completed the questionnaires. The mean age of the caregivers was 43 years old. The mean score for satisfaction with care was 3.80 (SD = 0.51) and the mean score for supportive care needs was 49.64 (SD = 20.49). The bivariate analysis found that four variables showed significant associations with supportive care needs. They were age of caregivers (r = -.24, p < .01), educational level (t = -3.27, p < .01), relationship with patients (F = 5.56, p < .01), and living status (F = 3.28, p < .05). Satisfaction with care and supportive care needs found had a significant correlation (t = -.33, p = < .01). A hierarchical multiple regression analysis showed that age of the caregivers (β = -.14, t =-2.27), relationship with the patients (β = -.18, t = -2.38), living status (β = -.20, t = -2.24), educational level (β = .23, t = 3.54) and satisfaction with care (β = -.26, t = -3.53) significantly determined supportive care needs, accounted for an adjusted R2 of .231. After controlling for the demographic variables, satisfaction with care explained 6.2% of the variances in supportive care needs.
Conclusions: The study found that satisfaction with care significantly correlated with supportive care needs. Younger age of the caregivers, relative of the patients, not living with the patients, lower educational level, and lower satisfaction with care determined higher supportive care needs. In the future, the assessment towards satisfaction with care and supportive care needs among caregivers of patients with AHF should be applied in hospital care.
