研究背景:
婦女在自然產待產時,生產疼痛是其最害怕的經驗。雖然婦女在產前教室可學習到拉梅茲等呼吸方法以減輕生產疼痛,但孕期婦女缺少在日常生活練習此呼吸法的機會。目前應用資訊科技提供周產期婦女照護已是一種趨勢,然而未見針對孕產婦待產期間緩解產痛的應用程式。
研究目的:
本研究旨在結合智慧型裝置,設計待產時拉梅茲呼吸方法應用程式(APP),探討孕期婦女運用此應用程式,對待產時生產疼痛、生產自我效能與生產滿意度的成效,以及使用此應用程式的滿意度。
研究方法:
本研究採隨機實驗設計,以方便取樣方式,於南部某醫學中心產科門診選取符合收案條件之孕期婦女,包括第一胎、計畫自然生產、非高危險性妊娠、懷孕30-34週。初次收案88人,婦女們以塊狀排列隨機分配方式,平均分派於實驗組44人,控制組44人。兩組婦女進行生產自我效能前測後,實驗組婦女給予研究介入措施「拉梅茲呼吸方法應用程式,和「拉梅茲呼吸方法衛教單張」於孕期使用;控制組婦女僅提供「拉梅茲呼吸方法衛教單張」。實驗組婦女每次門診產檢會進行回覆示教,兩組婦女皆於產後24-48小時進行後測,生產自我效能、生產疼痛視覺類比量表生產滿意度。此外實驗組婦女另填寫「呼吸應用程式使用滿意度」。研究結果以SPSS 19.0進行描述性統計、t檢定、One-Way ANOVA、Two-Way ANOVA及ANCOVA等進行分析。
研究結果:
本研究共51位婦女(實驗組23位,控制組28位)完成收案,平均年齡26.86歲 (年齡範圍20-37歲)。實驗組於第一、二產程平均時間為11.05小時;控制組為12.56小時,無統計上顯著差異(X2 = - .90,p = .37),期間有以硬膜外麻醉執行減痛分娩者佔43.1%。研究結果實驗組婦女顯著較控制組婦女於待產期間會使用呼吸法來緩解生產疼痛(X2 = 5.21,p = .02);當控制前測生產自我效能,使用「拉梅茲呼吸方法應用程式」之婦女的生產自我效能有顯著優於控制組婦女(F = 14.82,p < .001);兩組婦女在生產疼痛及生產滿意度無顯著成效;實驗組婦女對此應用程式整體使用滿意度為78.3%。
結論/實務應用:
本研究顯示使用「拉梅茲呼吸方法應用程式」可提升婦女生產自我效能,且能增加婦女能於待產期間執行呼吸方法頻率。建議此應用程式可結合產前衛教課程,提供婦女及家屬在產前衛教時更豐富的資訊及選擇,自我練習呼吸減痛技巧,增加生產時產婦能執行呼吸法以減緩生產疼痛,進而提升整體孕期照護品質。
The study aimed to examine the effectiveness of a Lamaze breathing skills application (LB-APP) on labor pain, childbirth self-efficacy and childbirth satisfaction. This study applied a randomized controlled trial (RCT) design, including an experimental group (EG) and a control group (CG). At trimester pregnancy, the pregnant women in the EG received an intervention, the LB-APP; whereas the women in CG received a breasting leaflet. A total of 51 pregnant women participated in study (experimental group = 23; control group = 28). The women in EG were more likely to practice breathing during labor than those women in CG. After controlling for pretest scores of childbirth self-efficacy, women in experimental group had significantly higher score of childbirth self-efficacy than that in control group (F = 14.82,p < .001). There were no group differences in labor pain and childbirth satisfaction. This study reveals the Lamaze breathing skills APP can enhance childbirth self-efficacy among perinatal women.
