  • 研究目的: 對於老年人來說,健康狀況變差以及日常生活需他人協助都可能成為一個人生命中的「逆境」,經過這些逆境後大多都會使其生活滿意度及幸福感下降。世界衛生組織指出「復原力」是一項老年人的重要關鍵資源,這項能力可使其在老化的過程中適應並有更多的掌控能力,並強調長期照護新思維已將核心目標轉變為 「使功能發揮做最大化」,以正向角度看見老年人的優勢與自身的能力。然而,檢視目前台灣長照 2.0 照顧管理評估量表內容多著重在個案之疾病或是需協助的地方,對於個案之復原力及內在能力尚未被強調。目前國內針對老年人接受長期照顧之復原力相關研究著墨較少,且過去多關注於機構住民,少有接受非機構式長期照顧老年人之研究。因此,基於復原力在長照服務提供的重要性,本研究擬探討在長照2.0之評估與照護計畫執行下,影響使用長照服務老年人之復原力因子為何,其復原力程度是否與生活品質有關係以及探討失能老年人與無失能老年人復原力及其影響因子之差異性。
    研究方法: 本研究採橫斷式研究法設計,以使用長照服務失能老年人與社區無失能老年人為研究對象,採用CD-RISC-10復原力量表、WHO-5生活品質量表、CES-D憂鬱量表和多向度社會支持量表進行測量。抽樣方法上,社區無失能老年人抽樣方法採便利抽樣;使用長照服務個案則採滾雪球抽樣。兩組收集問卷後進行資料分析比較。
    研究結果:兩組有效樣本共205人,控制社會人口學變項後顯示:(1)兩組之生活品質無顯著差異;(2)復原力與憂鬱程度呈現負相關,並與生活品質、家人/親戚社會支持呈現正相關;(3)復原力在心理健康狀況與生活品質之間具有中介作用,推測復原力可能可以作為老年人心理憂鬱及生活品質之保護因子。
    研究結論: 隨著年齡增長,可能會增加老年人失能的風險,而擁有復原力則能夠協助一個人正向的適應未來的生活壓力事件,故提供長照服務時,不該只將照護目標放在協助老年人維持生活,而必須也思考提高其內在能力,透過增強能力行為,重視老年人生理、心理及社會個別化需求並介入,提高個人能力並建構復原力,除了能避免照護依賴,同時也能提升其生活品質。

    This study set out to explain how resilience and quality of life among older adults utilizing long-term care services related to one another. The research objects are older adults using long-term care services and those who are not disabled in the community. In total, 205 samples aged 65 and over were assessed. The participants completed questionnaires that assessed their physical functions, depression, resilience, social support, and quality of life. Resilience was negatively correlated with depression level and positively correlated with quality of life and family/relative social support, suggesting that there was no significant difference in quality of life between the two groups. Resilience acts as a bridge between one's mental health and one's quality of life.
