水針又稱穴位注射，有別於傳統針灸，是中西醫結合的一種改良式療法。根據所患疾病，選擇相對應的穴位和藥物，將藥物注入穴位內，利用針刺的機械作用和藥物對穴位的刺激及藥物的藥理作用，發揮穴位及藥物對於疾病的綜和作用，進而達到治療或紓緩疾病的效果。目前水針療法廣泛的被運用於臨床上治療如:骨骼肌的損傷、關節炎、不孕症、腹瀉等，並且被報導有增強免疫力之功效。但針對水針療法其作用機制卻依舊不明，本實驗使用臨床上常見藥物來探討水針療法可能之機制。本實驗選擇足三里作為穴位，足三里為足陽明胃經的第36個腧穴，其具有明確的解剖位置，於臨床上被廣泛運用治療腸胃道疾病。實驗中利用神經細胞活性的指標，c-fos陽性反應的神經細胞，對於不同刺激反應在中樞神經系統中的表現量來定義神經活化的程度。實驗分為注射0.2ml蜜蜂毒素、維他命B1、維他命B12、生理食鹽水，以及0.6ml的維他命B1和生理食鹽水等六組。在大鼠左側足三里以水針注射0.2ml蜜蜂毒素，做為水針刺激的正對照組，結果顯示，c-fos陽性反應的神經細胞明顯表現於刺激同側邊的脊髓L2～L4節段，且主要分布在脊髓灰質背側角第I、II、V層;在I、II層處較明顯聚集於中間。注射0.2ml的維他命B1組，其與蜜蜂毒素組有相同的趨勢，均明顯表現於刺激同側邊的脊髓L2～L4節段且同側邊相較於對側邊有顯著的差異。注射0.2ml維他命B12與生理食鹽水組，在其刺激的同側邊與對側邊的脊髓各節段c-fos的表現量均無差異。在注射0.6ml維他命B1及生理食鹽水組，在水針施打同側L2～L4之脊髓灰質背側角，相較於0.2ml的組別有顯著的增加。結果顯示，注射蜜蜂毒素及維他命B1，其機制與神經活化有關聯。此外，將注射液的體積提高至三倍，我們發現增加體積可以加強水針的刺激強度。綜合以上實驗結果，我們推論注射液中不同的藥物成分所產生的療效或刺激，並非透過單一機制，而注射液的體積也可能為水針刺激強度之影響因子。

Aquapuncture is defined as a therapeutic method in which an injectable agent is applied to the acupoint. It is an effective and alternative method comparing to traditional needle puncture. Effect of aquapuncture combined needle stimulation and pharmaceutical agent. In recent years, clinical therapy of aquapuncture is used extensively for the treatment of musculoskeletal injuries, arthritis, infertility, diarrhea and improved immune responses have been observed, but the mechanism of aquapuncture is still uncertain. Purpose of our study is to investigate the curative effect of aquapuncture with different agent and the possible mechanism of aquapuncture. The Zusanli (ST-36) acupoint was chosen in this study, for it is widely used in gastrointestinal disease and a defined anatomical location and innervation. Mapping c-fos expression is a valuable tool for identifying neuronal activation in the central nervous system in response to peripheral sensory neuronal stimulation. In this study, substances used for injection are 0.2ml 1% bee venom (BV), vitamin B1 (vitB1), vitamin B12 (vitB12), 0.9% normal saline (NS), and 0.6ml vitB1 and NS. After 0.2ml BV injection into Male Sprague-Dawley rats left Zusanli as positive control. A large number c-fos expression neurons were found at spinal dorsal horn of ipsilateral lumbar (L2～L4) segments. Densely labeled neurons were spread in lamina I, II, V, and c-fos positive cells distributed preferentially in the middle one third of lamina I, II. We treat aquapuncture with 0.2ml vitB1, into rat left Zusanli resulted in the similar pattern. There are no significantly c-fos expression neurons in spinal dorsal horn after 0.2ml NS and vitB12 applied into Rat left Zusanli. More c-fos expression neurons were found at spinal dorsal horn of ipsilateral L2~L4 segments after 0.6ml vitB1 and NS treatment. These result indicated that aquapuncture using BV and vitB1 solution through the mechanism is involved in neuronal activation. In addition, triple injection volume even NS could enhance the aquapuncture stimulation. These results suggest that the curativeness of different pharmaceutical agent in the injectable solution via not only one kind of mechanism and injection volume could be possible effective factors of aquapuncture.
