本篇論文探討台灣全民健康保險之部分負擔政策改變對於民眾就醫需求之影響，並利用投保身份類別代表不同社會經濟地位之病患，進一步探討部分負擔政策對於不同社會經濟地位之病患的醫療院所階層選擇。本研究利用2010年百萬抽樣歸人檔之全民健保研究資料庫與所需外部資料，以投保身份別定義社會經濟地位，並以1997年至2010年之門診病患分為總樣本、不包含依附加保眷屬之樣本以及投保身分別為第一類至第三類之樣本，利用最小平方線性模型與負二項分配固定效果模型做分析。總結本文之結果，發現歷次部分負擔政策的改變可以在短期有效抑制民眾的醫療需求，但往往政策的效果只維持一段時間，之後又會呈現上升之趨勢。因此得知，從需求面的政策是無法有效達到控制醫療需求之目的，需透過供給面政策之配合，方能達到有效的控制醫療需求的增長。此外，本文以該病患就醫診次最多次的醫院設定為負責醫療機構，結果顯示不管是公立或私立醫療機構，高層級醫療機構診次高於低層級醫療機構診次，也就是若以較高層級醫療機構為負責醫療機構，則因品牌與忠誠度效應，其門診次數相對較高。最後，私立診所診次高於公立診所，其因在於台灣診所大多數為醫師自行開業所致。

This paper examines the effect of change in co-payment policy on medical demand of outpatient and the selection of different socioeconomic status of patient for hospital in Taiwan. Based on the one-million individual sampling claim data selected randomly by the registry of National Health Insurance beneficiaries in 2010, we use outpatient claims data from 1997 to 2010 as our sample. By taking different types of insurance identity as socioeconomic status of patient, and we divided sample into three sub-samples: the total sample including six insurance identity types, the sample without dependents and the sample for insurance identity from type one to type three. We used ordinary least square and negative binomial fixed effects regression model to do our analysis. Overall, we found that once implementing co-payment policy it would significantly decrease medical demand of outpatient services. But the effect of change in co-payment policy lasted about one year, and then medical demand bounced back d again.
