  • 背景:護理人員低專業生活品質與高盛行率之同理疲憊感(疲潰與二次創傷壓力)與低盛行率之同理滿足感有關。
    目的:本研究目的在探討護理專業生活品質以及其與護理人員之專業價值、道德困境、同理疲憊感與自我關懷之關係
    方法:本研究為相關性研究,採橫斷式問卷調查法收集資料。研究對象包含印尼中爪哇三所不同層級醫院護理部門之所有護理人員。使用群集隨機抽樣法共招募600位護理人員接受調查。研究工具使用5個量表包括:護理專業價值量表3-印尼版(NPVS3-I)、同理疲憊量表修定版(MDS-R)、自我關懷量表(SCS)、關懷能力量表(CCS)、專業生活品質量表5版(ProQOL 5)。相關資料以Pearson相關性與多元迴歸進行統計分析。
    結果:參與者七成為女性(n= 416),平均年齡為33.24±7.54歲。專業生活品質量表(ProQOL)各向度平均分別為中等程度同理滿足(CS,37.91±5.44;74.80%)、中等程度之同理疲憊(CF,49.15±11.68;51.70%)、中等程度之疲潰(BO,22.94±5.04;56.00%),以及中等程度之二次創傷壓力(STS,26.21±7.78;53.85%)。結果發現同理滿足與護理專業價值具統計正相關(NVPS3-I,r= 0.083, p < 0.05),且與同理疲憊量(MDS-R,r= 0.090, p < 0.05)有正相關。而自我關懷量表(SCS)與同理疲憊具負相關(CF,r= -0.281, p < 0.01),與疲潰具負相關(BO,r= -0.231, p< 0.01),並與二次創傷壓力呈現負相關(STS,r= -0.284, p < 0.01)。關懷能力量表與同理滿足呈負相關(CS,r= -0.265, p < 0.01),但與疲潰(BO,r= 0.136, p< 0.01)呈正相關。
    多元迴歸分析發現子女數只有2個以下較超過2個的有較高的同理滿足(B= 1.926, p < 0.01);薪資> 800,000 ~ 1,500,000 IDR較 > 4,000,000 IDR者有較低的同理滿足(B= -2.670, p < 0.05)。護理價值可預測較低的同理滿足(B= - 0.100, p < 0.01);關懷可預測較高的同理滿足(B= 0.343, p< 0.01);自我關懷可預測較高的同理滿足(B= 1.693, p < 0.001);孤立可預測較低的悲憫滿足(B= -0.721, p < 0.05)。
    全職工作可預測較高的同理疲憊(B= 5.484, p < 0.001),每日工作時間 < 8小時的護理人員有較低的悲憫疲憊(B= -2.290, p < 0.05);自我關懷可預測較低的同理疲憊(B= -3.050, p < 0.05);自我判斷可預測較低的同理疲憊(B= -2.870, p < 0.01)。
    有全職工作的護理人員有較高的疲潰(B= 1.805,p< 0.01);自我關懷可預測較低的疲潰(B= 5.484, p < 0.001);自我判斷可預測較低的疲潰(B= -1.405, p < 0.01)。
    有全職工作的護理人員有較高的二次創傷壓力(B= 3.354, p < 0.01),每日工作時間<8小時的護理人員有較低的二次創傷壓力(B= - 1.767, p < 0.05)。
    結論:綜整研究結果發現影響印尼護理人員護理專業生活品質之預測因子包括:護理人員子女數量、護理價質、關懷、自我關懷、自我判斷、每天工作時數與全職工作。

    Background: The low professional quality of life in nurses can be seen from the high prevalence of compassion fatigue (burnout and secondary traumatic stress) and low prevalence of compassion satisfaction.
    Objective: This study aims to explore nurses' professional quality of life and determine the correlation between professional quality of life with professional values, moral distress, and compassion among nurses.
    Method: This study is a correlational study using cross-sectional survey design. The study was conducted at three hospitals in Central Java, Indonesia involved nurses in all levels and departments. Data collected from 600 nurses using a cluster random sampling technique. Measurements including five scales; the Nurses Professional Values Scale-3 Indonesian version (NPVS3-I), the Moral Distress Scale-Revised (MDS-R), Self-compassion Scale (SCS), Compassion Competence Scale (CCS), and Professional Quality of Life 5 (ProQOL 5). Pearson correlation analysis and multiple regression analysis were performed for data analysis
    Results: The participant were mostly female (n = 416, 70.00%) with age ranged 33.24±7.54 years. The ProQOL mean score for compassion satisfaction (CS), compassion fatigue (CF), burnout (BO), and secondary traumatic stress (STS) were 37.91±5.44, 49.15±11.68, 22.94±5.04, and 26.21±7.78, respectively. The ProQOL level of each domain were moderate for CS (74.80%), moderate for BO (51.70%), and moderate for STS (56.00%). The level of CF was moderate (53.85%). The results indicated that NVPS-I have a positive correlation with CS, r = 0.083, p < 0.05. MDS-R have a positive correlation with CS, r = 0.090, p < 0.05. SCS have a positive correlation with , CF, BO, and STS, -0.281, p < 0.01, r = -0.231, p < 0.01, and r = -0.284, p < 0.01, respectively. CCS have a negative correlation with CS, r = -0.265, p < 0.01 and positive correlation with BO, r = 0.136, p < 0.01, respectively. Multivariate regression correlation showed some predictors are correlated with the professional quality of life. Having 1 to 2 children compared to having > 2 children predicted higher CS (B = 1.926, p < 0.01), nurses’ salary > 800,000 to 1,500,000 IDR compared to nurses’ salary > 4,000,000 IDR was predicted lower CS (B = -2.670, p < 0.05), nurses’ professional values was predicted lower CS (B = -0.100, p < 0.01), caring was predicted higher CS (B = 0.343, p < 0.01), and self-kindness was predicted higher CS (B = 1.693, p < 0.001) and isolation was predicted lower CS (B = -0.721, p < 0.05). Nurses who work with full-time job was predicted higher CF (B = 5.484, p < 0.001), nurses working time per day < 8 hours compared to ≥ 8 hours per day was predicted lower CF (B = -2.290, p < 0.05), self-kindness was predicted lower CF (B = -3.050, p < 0.05), and self-judgment was predicted lower CF (B = -2.870, p < 0.01). Nurses who work with full-time job was predicted higher BO (B = 1.805, p < 0.01), self-kindness was predicted lower BO (B = 5.484, p < 0.001), and self-judgment was predicted lower BO (B = -1.405, p < 0.01).     Conclusion: We found the predictors of the professional quality of life among nurses in Indonesia are nurses’ number of children, nurses’ professional values, caring, self-kindness, self-judgment, isolation, working time per day, and a full-time job.
