  • 近年來台灣社會除了關注疾病之預防及治療等議題,相關之營養問題也逐漸受到重視,但大多數醫院並不容易提供適當之營養照護。臨床營養師進行營養照護時,通常都以紙本紀錄,不僅需要大量人力,而且浪費紙張,紙本病歷也較為厚重,不方便調閱資料以供臨床營養師查詢。因此,若能提供臨床營養師一個便利之資訊工具,將帶給臨床營養師很大的幫助。
    另外,臨床營養師通常會參考營養診斷並依據病患個人之疾病、成長、教育和所處環境狀態及社會經濟等問題,給予適當之營養介入,以提昇照護品質。因此,本研究以病患個人化營養評估為研究方向,並進行資訊系統開發。
    在和某醫學中心多位臨床營養師訪談需求之後,本論文開發的營養資訊系統提供各種營養篩檢需要之工具,以適用於各種不同病症之病患。例如,針對病患提供快速自動計算營養相關指標數值,記錄熱量與蛋白質攝取量,產生需求量及建議量;並於營養紀錄中整合病患各種生理數據,透過臨床營養師專業評估及營養診斷資料庫之協助,即時產生適合該病患之營養衛教單張及飲食建議等個人化營養評估。另外,亦提供多樣報表及追蹤紀錄,以協助臨床營養師針對各種需求進行資料統計,並瞭解病患長期照護之後的營養趨勢及改善情況。
    透過問卷調查統計,有99%的臨床營養師對於本系統整體上是感到滿意,說明本系統是可用的。有85%的臨床營養師肯定本系統設計之主要功能,確實可以給予實質上的幫助,並節省了約55.7%的作業時間,提昇工作時間效率。

    In this study, a clinical nutrition information system is designed and implemented. The system is to help nutritionist to perform nutrition care. The major functions are to support the entrie patient’s care process, and to generate nutrition handout forms and required statistics.
    The new feature proposed in this study is “personalized”. More personalized information is provided to help a nutritionist to make a better nutrition plan for a patient. After interviewing many clinical nutritionists in a medical center, we have a clear requirement for the system. The system provides many nutrition tools for evaluating a patient’s nutrition status. For instance, based-on the intake records, it rapidly generates nutrition indicator values, nutrition demand and proposed calorie and protein for a patient. In addition, it integrates patient physiological data in nutrition records, and it can instantly show appropriate personalized nutrition handout forms of nutritional assessment and dietary recommendations after a nutritionist’s professional assessment and nutrition diagnosis. It also assists the nutritionists to generate statistical reports and trace records, and to help patients who are taking long-term care.
    A survey on the satisifiablity of the proposed system is performed. Around 85% nutritionists agreed that the system really helps their work, and saves 55.7% processing time.
