為了提升長期照顧機構的照護品質，政府制定了許多新的評鑑標準，該如何為老年人選擇合適的長期護理機構是值得受到關注的。然而，大多數研究集中在機構設施的硬體和軟體上面，而忽略了老年人的心理狀態，導致入住後出現許多適應問題。
本研究著重於中老年人入住機構時的決策過程，旨在透過自我決定理論探索他們的心理需求，此研究結果有助於長期照護機構了解老年人的入住過程並改善入住後之生活品質。
研究方法: 本研究採用方便取樣方法進行南台灣長期照顧機構住民問卷調查。收案對象為剛入住三到六個月的老年人，使用結構式問卷進行一對一訪談，問卷中客觀健康狀態包括:慢性病罹病狀態、日常生活自理能力(ADL,Barthel index)、認知功能(MMSE、SPMSQ)、健康相關生活品質量表(EUROQOL,EQ-5D)。總共收案50 個機構（包括養護中心和護理之家）共計200個樣本的有效問卷。
研究結果：本研究樣本為長照機構老年人，平均年齡大多落在71~80 歲，有116 位為女性、84 位男性，均有罹患慢性病(100%)，其中罹患4 種慢性病者最多有81 人(40.5%)，ADL 平均分數46.01±23.09(22.92-69.1)，經階層式迴歸分析發現如下:在行為變項加入後老年人的健康狀況是決定入住機構的重要因素;日常生活自理能力(ADL,Barthel index)為生活品質之影響因子;從自我決定理論的角度來看，勝任感是影響長期照護機構中老年人的生活品質的顯著因子。
結論: 基於基本人權的尊重與照顧安全的考量，本研究亦關注選擇長期照顧機構的決定過程中，高齡者的心理需求運作狀況，主要是希望從高齡老年人心理需求的角度，重新省思老年人自我決定權的議題，並避開疏忽與過度強調自主權的陷阱，真正達到案主自決與協助案主追求利益的社會福利服務之目標。

SUMMARY
This study aimed to examine the psychological needs of older adults and its relationship with their quality of life in institutions by using the self-determination theory. The research method is cross-sectional by using convenience sampling method to conduct a person-to-person interview with residents receiving instructional care in Taiwan. The psychological needs scale and the EQ-5D were measured and Hierarchical Linear Regression was conducted for data analyses.
