  • 目的:後天性腦傷 (Acquired brain injury) 指的是發生於出生後之任何類型腦部傷害。腦傷受損機制複雜,其後遺症將對患者之認知、情感、社會、職業、家庭功能及生活等多個層面產生明顯且長期之影響。過往研究顯示家庭內主要照顧者參與程度對於患者的疾病預後有百分之四十的解釋力,顯示家屬是疾病適應歷程中的關鍵因素。然過去研究多侷限在患者家屬之負向情緒與壓力,很少以正向觀點探討家屬成功適應疾病之具體機制。腦心智功能已被確定是影響個體適應結果的重要因素,因此,本研究以腦心智功能觀點出發,探究家屬適應疾病之具體機制,並建構家屬面對疾病適應歷程的本土化架構,以提供腦傷患者家庭面對困境時具體可用之心理策略。

    方法:本研究經臺灣南部某醫學中心人體試驗委員會審核通過,編號A-ER-110-145,採立意取樣招募符合條件之後天性腦傷患者家屬參與,並於具隱密性、安靜、不受打擾的環境進行半結構式深度訪談,訪談結果以紮根理論進行資料整理與分析。

    結果:在8份文本資料中共標定出1012筆意義單元 (全距為73至183),將文本資料以紮根理論進行整理與分析後,共發展出882筆概念化、288筆次範疇、35筆主範疇、9個面向及4個核心軸。四個核心軸分別為腦心智功能軸、資源系統軸、醫療歷程軸及社會文化軸。

    討論與結論:家屬面對後天性腦傷疾病之適應歷程受多項因素影響,其中腦心智功能是影響疾病適應結果的重要因素,家屬在各個疾病階段若能運用自身腦心智功能的優勢以因應當前挑戰,便能得到較佳疾病適應結果。此結果可作為未來臨床神經心理領域介入之方向。

    Objective: The past studies are limited to the stress and the emotional disturbance of the family caregivers. However, they rarely investigate the positive adaptation mechanism from the perspective of neuropsychological functions. This study intends to construct a neuropsychological framework for families to adapt to acquired brain injury (ABI).

    Method: This research focuses on family caregivers' disease adaptation to the patient with ABI. Intentional sampling and the semi-structured in-depth interview method are applied to the eligible participants. Furthermore, all interviews are conducted in a private, quiet, and undisturbed space; the grounded theory research method is used to analyze the data.

    Result: A number of 1012 units are calibrated from eight datasets. After sorting, analyzing, and summarizing with grounded theory, 882 concepts, 288 subcategories, 35 categories, nine aspects, and four cores are labeled.

    Discussion: Family caregivers' disease adaptation to ABI is affected by several factors. Among factors, neuropsychological functions are crucial to the disease adaptation outcome. Family caregivers who are able to use the advantage of neuropsychological functions to cope in all stages of ABI may have better disease adaptation outcomes. The result can be used as a direction for future intervention in the field of clinical neuropsychology.
