中風患者手的損傷是造成日常活動限制的主要原因,有效恢復中風病患手部功能是有迫切性的。近年來研究指出震動刺激的介入是有效的提升中風病患上肢動作復健的效果的一種方式,且日常生活任務型動作能增進病患皮質活動進而讓動作能力恢復更有效。本研究採用個案研究的方式,目的為探討震動刺激結合日常生活任務型動作是否能有效增進慢性期中風病患手部動作能力。
本研究的個案研究,納入的2位慢性期中風病患分別為初次中風至今約6年的51歲男性,與中風至今約7.7年的75歲女性,兩位皆為左側偏癱,實驗介入前患側手部不能完全獨立動作。病患接受每次30分鐘,每週2次,連續6週,共12次的震動刺激療程,療程中震動刺激搭配任務型動作。患側手部功能的評估方法以捏力檢測與盒子積木測驗良量測病患手部精細動作能力、傅格-梅爾評估量表來評估整體手腕手部動作能力、與動作活動日誌以評估患側手部在日常生活中的動作情況。資料的蒐集除了評估數據,還有深度訪談與觀察,於實驗療程期間對病患進行半結構式訪談,訪談目的為了解病患對接受療程後,主觀發現的手部功能與日常生活手部動作的改變、和參與療程的心境變化,每周的訪談結果會由研究人員記錄,並與以參與者一同驗證。以訪談的數據來深入探討病患主觀對手部動作功能的改變,與評估量表結果的關係。
研究結果顯示,2位病患皆於手部精細動作能力的捏力測驗與盒子積木測驗的評估結果顯示上有進步,傅格-梅爾評估量的評估結果則在1位病患身上看到進步。而在日常活動中患側手部動作執行狀況的動作活動日誌結果中,與療程介入前相比,2位病患皆沒有顯示有差異。另外於訪談結果,2位病患皆於療程介入後指出自己發現於執行療程動作與日常生活動作中的手部動作進步。
本研究結果證實震動刺激與任務型動作訓練可能有益於改善慢性期中風患者的手部精確動作能力,並任務型動作訓練或許能提升中風病患日常生活動作能力。
The impairment of hands following a stroke has a potential impact on activities of daily living. Therefore, it is exigent for stroke patients to improve the motor function of their hands. Many recent studies show that vibration stimulation is an effective way to improve hand motor function recovery. Additionally, studies indicate that task-oriented training can increase the cortical activity and may induce greater motor recovery. Hence, the purpose of this case study is to evaluate the effect of vibration stimulation combined with task-oriented training on the motor function of hands in chronic stroke patients.
Two post-stroke patients participated in this study, one 51-year-old man who had suffered from a stroke for 6 years and one 75-year-old woman who suffered from a stroke for 7.7 years. The patients received the vibration stimulation with task-oriented training for 30 minutes a session with a total of 12 sessions over a 6 week period. The delicate controls of hand motor function of the patients were assessed by Box and Blocks Test and pinch force detection, and the Fugl-Meyer assessment for measuring the motor functions of the wrist and hand. A Motor Activity Log (MAL) was used to measure how much and how well the patients had used their affected hand for activities of daily living. In addition to the data of the clinical evaluation tests, the researcher conducted semi-structured interviews to understand their experiences receiving the treatment, the changes they noticed in their hand movement during the treatment and in their daily activities to investigate the relationship with the evaluation results. The interview data were recorded by the researcher every week, and were verified with the patients.
The evaluation results of Force ratio and BBT showed the improvement in their delicate control of hand motor function, and the results of the FMA showed improvement in one patient. According to the interview, both patients found changes in their hand movements during the task-oriented training and in their activities of daily living.
The results of the study showed that the vibration stimulation combined with task-oriented training may be able to enhance the motor abilities of hands, and the task-oriented training may be able to transfer the training movements to the activities of daily living.
