  • 本研究目的為探討放鬆介入對化療期罹癌患者之生理焦慮、心理焦慮和行為焦慮之影響及化療期罹癌患者之生理焦慮、心理焦慮和行為焦慮之關係。以40位南部某醫學中心接受第一或二次接受化學治療之罹癌患者為研究對象,並隨機將之分為放鬆介入之實驗組或常規照護之控制組。焦慮指標的的評價包括生理焦慮指標(皮膚電阻與指溫)、心理焦慮指標(情境焦慮與視覺焦慮)和行為焦慮指標(臉部表情與清醒-睡眠狀態)。統計分析各變項前後測得分之兩組比較採獨立樣本t檢定與卡方檢定、連續變化之比較採線性混合模式,及關係探討則採階層回歸加以分析。結果:(1)生理焦慮指標:實驗組自第5-10分鐘開始指溫組內有明顯的增加,而控制組無,而皮膚電阻兩組間則無顯著差異;(2)心理焦慮指標:情境焦慮與視覺焦慮兩組皆有明顯改善(t= -.900,p= .374; t= 1.283, p= .207);(3)行為焦慮指標:清醒-睡眠狀態自第10、20或30分鐘開始兩組間達統計顯著(χ2(1)= 5.227, p= .022;χ2(1)= 4.552, p= .033;χ2(1)= 7.111, p= .008);(4)情境焦慮後測得分主要與情境焦慮前測、放鬆介入與否、第15分鐘與第25分鐘的皮膚電阻有關。本研究結果顯示罹癌患者在放鬆介入下可有效改善生理和行為焦慮指標,然而本研究由於樣本數少,建議未來可擴大樣本數以檢視此論點,藉以提供對具化療期之罹癌患者更精緻之照護。

    The purpose of the study was to explore the effects of relaxation intervention on physiological, psychological and behavioral anxiety reduction for cancer patients during chemotherapy and the correlation of physiological, psychological and behavioral anxiety. The study subjects included 40 cancer patients who had undergone first or second chemotherapy. They were randomly assigned to relaxation intervention or control group. The assessment included physical anxiety index (galvanic skin response and finger temperature), psychological anxiety index (state anxiety and visual anxiety) and behavioral anxiety index (facial expression and wake-sleep status). Independent samples t test and Chi-square test were used to compare the group differences in pre and post test and linear mixed model was used to exam continues data. Results indicated that (1) physical anxiety index: finger temperature was increased significantly in intervention group 5-10 mintumes after intervention, but no significant differences between groups was found in galvanic skin response; (2) psychological anxiety index: state anxiety and visual anxiety were improved significantly in both intervention and control group (intervention t= -.900,p= .374; control t= 1.283, p= .207); (3) behavioral anxiety index: wake-sleep status from 0 to 10, 20, or 30 minute after intervention was improved significantly in the intervention group ( χ2(1)= 5.227, p= .022;χ2(1)= 4.552, p= .033;χ2(1)= 7.111, p= .008). (4) pre-test state anxiety score, the relaxation intervention, 15 mintumes and 25 mintumes galvanic skin response were a significant predictor post-test state anxiety score. The result of this study revealed that relaxation intervention was more effective in objective index (physical anxiety index and behavioral anxiety index). However, this study contained limited sample and afterwords larger sample size is recommended for future study.
