眾多研究已證實哺餵母奶的好處,由於乳房與奶嘴在結構上的差異,親餵時能刺激乳汁分泌增加母乳量及增進自己與孩子的依附關係,更能讓母親感到自己對孩子的重要性,且對於早產兒而言,親餵較瓶餵少發生呼吸暫停、低血氧飽和濃度及口鼻周圍膚色改變的情形,晚期早產兒為出生週數34-36週新生兒,儘管週數接近於足月兒,但因生理未成熟,仍存在早產的疾病風險,無法如足月兒在出生時或是出院時親餵母乳,及臨床上部分專家仍認為親餵容易造成早產兒疲憊,使得早產兒接受親餵與否受到複雜因素影響,如:早產兒疾病合併症、母親過去哺乳經驗和社會支持等之因素,這些影響早產兒母親親餵決定因素當中,很少研究探討晚期早產兒母親親餵決策因素及重要預測變項。故本研究目的探討晚期早產兒母親親餵決策及其相關因素。
本研究以橫斷面調查研究設計,採立意取樣,納入符合條件之晚期早產兒及其母親,於晚期早產兒矯正年齡40週時,以結構式問卷調查66位晚期早產兒母親,探討母親親餵決策包含親餵與否及預計持續親餵時間。
以邏輯斯迴歸分析發現產前親餵計畫 (odds ratio = 6.60, p = .024)、母乳哺餵自我效能 (odds ratio = 2.47, p = .015)、醫護人員母乳哺餵社會支持 (odds ratio = 1.66, p = .037) 可預測晚期早產兒母親親餵決策的重要預測因子。希望藉此研究於臨床上能事先了解母親產前有無親餵計畫,在母親產前至產後階段,醫護人員所提供相關的母乳哺餵照護及支持,則可促進母親母乳哺餵的意願及持續性,提升母親母乳哺餵的自信心,進而增加其親餵的意願。
The physiological immaturities of late preterm infants place them at risk of neonatal complications; thus, feeding at breast is challenging for the mothers of late preterm infants. The present study aimed to explore the related decision-making factors for feeding at breast among the mothers of late preterm infants. A cross-sectional study was conducted in two hospitals in southern Taiwan between 2019/4-2019/10. A total of 66 mothers of late preterm infants participated and the results showed breastfeeding plan prior to pregnancy, higher breastfeeding self-efficacy and professional breastfeeding support were important predictors for the mothers to decide whether feeding at breast for their late preterm infants. This study recommends that health care providers should continue to promote and support breastfeeding during prenatal and postnatal period to assist mothers in formulating their breastfeeding plan and increase their self-efficacy of breastfeeding to increase their willingness to feed at breast for their baby.
