台灣每年有300至400個唐氏兒出生,但70%至80%的唐氏兒都是由年輕懷孕婦女所生。目前國內於34歲以下的懷孕婦女多採用母血篩檢作為初步篩檢的方式。母血篩檢對於外籍女性配偶而言,可能成為他們第一個碰到的遺傳檢測。在決定是否接受母血篩檢過程中,外籍女性配偶常是藉由先生的解釋,間接獲得母血篩檢的資訊。所以,外籍配偶接受母血篩檢的知情選擇是無法確定的。目前國內缺少關於外籍配偶知情選擇上的研究。本研究之目的是在探索越南籍配偶於母血篩檢之知情選擇上的情況並以台籍婦女作為對照。
本研究採回溯性研究,以方便取樣及滾雪球的方式選取懷孕14週以上或是生產過後2年內的越南籍及台灣籍婦女各83人。依據知情選擇多面向測量(MMIC)的概念發展出問卷,進行一對一訪談問卷資料收集。研究結果顯示(1)台灣籍婦女的知情選擇情形以及母血篩檢相關知識皆顯著高於越南籍婦女。(2)台灣籍婦女對於母血篩檢的態度與越南籍婦女皆呈現正向的態度。(3)台灣籍婦女的態度篩檢行為一致性為影響台灣籍婦女知情選擇最主要的因素,而影響越南籍婦女知情選擇的最主要因素為對於母血篩檢的相關知識。(4)知識並非預測婦女是否執行母血篩檢或是婦女態度篩檢行為一致性的重要因子。(5)在知識方面,超過50%的婦女並不了解母血篩檢可以自由選擇是否接受。本研究結果將可作為日後在母血篩檢流程上進行知情選擇流程制訂的參考依據。
Data were indicated that approximately 70% to 80% of the Down syndrome babies were delivered by mothers aged below 34 years old. Women in normal childbearing age are recommended to receive the maternal serum screening (MSS) to enhance the prenatal genetic health. Therefore, for those immigrated female spouse, the MSS would be their first genetic test while they were pregnant. Because of the language barrier, health professionals experienced great challenges to achieve of informed choice of MSS in immigrated pregnant women. In order to enhance these immigrated female spouses’ competent to perform informed choice of receiving the MSS, there was a need to understand the women’s informed choice of MSS. Due to the studies regarding the informed choice for women receiving MSS were few, the purpose of the study was to examine the informed choice of MSS in Vietnamese and Taiwanese women. A retrospective study design and the convenient sampling were used to examine the research hypotheses. Eighty-three Vietnamese female spouses and the same number of the Taiwanese women were recruited to participate of the study. The questionnaire based on the concepts of MMIC was developed by the researcher to collect the data. Results of the study show that (1)Taiwanese female spouses’ informed choice and knowledge of MSS were higher than those of Vietnamese women; (2)both of Taiwanese and Vietnamese women displayed positive attitude toward MSS; (3)The major predictors of informed choice of MSS in Taiwanese and Vietnamese female spouses respectively were the consistency of uptake and attitude of MSS and the knowledge of MSS; (4) knowledge of MSS was not the major predictor of the consistency of attitude and uptake the screening; (5) more than 50% of all the research subjects did not understand that MSS was not an obligated prenatal test. Lastly, results of the research could be adopted as the reference to formulate the standards of conducting MSS.
