  • 失智症患者因認知障礙及行為問題使得照護人員常面臨複雜之溝通情境,而長期照護機構之護理人員為與失智症患者溝通的主要對象,因此深入了解護理人員面臨的溝通困難及其對促進有效溝通的需求,以提供護理人員之失智症溝通教育之實證參考。護理人員若能接受專業溝通教育訓練,可提升與失智症患者溝通之敏感度,且能更了解患者的照護需求,進而改善照護品質。截至目前為止,在台灣關於失智症患者溝通之相關研究及論述仍較著重於教導護理人員與失智症患者溝通技巧,而忽略探究護理人員本身與失智症患者之溝通困難。本研究目的在探討護理人員與失智症患者溝通時所遭遇之困難、了解阻礙溝通的因素、並探知護理人員的需求。研究設計為探索性質性研究。運用深度訪談法,以立意取樣的方式,從南部數所長期照護機構中取得實際照顧失智症患者經驗之護理人員為研究對象,以內容分析法做資料分析,研究結果分三個範疇呈現八個主題:(一)溝通困難為雞同鴨講及難入內心;(二)阻礙溝通的因素為機構環境之限制、自我準備度不足、先入為主之觀感;(三)溝通的需求為經驗累積及同儕分享、知識再造與心靈成長、機構及外界之協助。研究推論因長期照護機構聚焦身體照護,使得護理人員無足夠時間而處於溝通困境之中,再加上護理人員本身對失智症溝通的經驗及學習有限,致遭遇溝通困難時,易產生退縮及迴避的反應而影響傾聽及回饋,因而降低可協助失智症患者獲得身心安適之溝通品質。期望本研究結果能提供未來護理教育與護理實務在規劃失智溝通相關教育計畫之實證參考。而長期照護機構能加強人力資源分配及補給、提供護理人員心靈成長及溝通問題的諮詢對象、改善溝通環境之硬體設置等,皆為提升護理人員與失智症患者溝通之要務。

    Because of patients’ language defect and behavioral problems, caregivers often face complexes when communicating with dementia patients. Nurses in long-term care facilities play a key role to communicate with dementia elders. Therefore, understanding nurses’ difficulties and demands in communicating with dementia patients become critical. If nurses could receive professional training in communication, their sensitivity to patients needs will increase, and so the quality of care may improve. Past dementia communication researches were mainly focused on intervention skill development; very few explored communication difficulties among nurses. Therefore, the purpose of this study was to explore nurses’ predicaments and demands in communication with dementia patient. Besides, factors affecting communication were examined. Exploratory qualitative research design using in-depth interview and purposive sampling were applied. Study subjects are nurses who have care experience in dementia elderly from several long-term care institutions in southern Taiwan. Data were analyzed using content analysis. Eight themes direct to communication difficulties, factors affecting communication, and future need were emerged respectively:(1) Difficulty in irrelevant communication and difficulty in approaching patient’s world;(2) Factors affecting with environmental limitation, self-preparation insufficiency, and prejudice;(3) Need of experience gain and peer sharing, knowledge restoration and spiritual growth, and assistance from institution and outside condition. Discussion of this study implies that physical care oriented environment in long-term care institutions impedes nurses’ effective communication with dementia patients which decrease well-being and care quality received among this population. Findings can be the basis when planning for future nursing curriculum and practice. Furthermore, increasing human resource allocation and replenishment, providing spiritual care and consultation services, improving environmental space and equipment are important tasks for effective communication between nurses and patients.
