  • 背景:緩解期雙相情緒障礙症患者的社會支持(主觀性與客觀性;物質性與情緒性)為社會功能的重要指標之一,且對於患者的症狀嚴重度有顯著的預測力,顯示探索患者社會支持程度之影響因子的重要性,然而此研究尚缺乏,本研究選擇社會認知功能與孤獨感兩種概念做為可能之影響因素。研究已證實緩解期患者的社會認知功能受損,但與社會功能關聯性的研究結果不一致;而緩解期患者的孤獨感程度尚未被研究證實,仍需更多研究驗證。因此緩解期雙相情緒障礙症患者之社會認知功能及孤獨感對於社會支持程度影響仍未有定論。
    目的:本研究欲瞭解緩解期雙相情緒障礙症患者的社會支持之影響因子,分別以客觀能力-社會認知功能及主觀感受-孤獨感進行探討。
    方法:本研究納入緩解期雙相情緒障礙症患者共59位,健康對照組共43位,以漢氏憂鬱量表與楊氏躁症量表分數均≤7分做為緩解期標準,並收集人口學變項,以社會功能量表測量社會支持程度,並以心智理論測驗及眼神讀心測驗測量心智理論能力、台灣版診斷性非語言行為準確性分析工具(DANVA-2-TW)測量情緒辨識能力,孤獨感則以UCLA孤獨感量表進行測量,並以相關及多元迴歸分析檢驗社會認知功能與孤獨感在社會支持上的影響。
    結果:緩解期雙相情緒障礙症患者在社會功能量表及其分量表得分皆顯著低於健康對照組。在社會認知功能上,緩解期雙相情緒障礙症患者在語文心智理論測驗及情緒辨識測驗上皆表現較差。在孤獨感上,緩解期雙相情緒障礙症患者的UCLA孤獨感量表得分顯著高於健康對照組。在相關及多元迴歸分析中,結果顯示健康對照組及雙相情緒障礙症組於心智理論測驗分數皆與主觀物質性社會支持有顯著相關性,而孤獨感與主觀情緒性社會支持的關聯性同樣可見於兩組。
    結論:緩解期雙相情緒障礙症患者有較低的社會支持程度、較差的心智理論能力與情緒辨識能力、較高的孤獨感。且兩組之心智理論能力越差皆較難感受或獲得他人所提供之物質性協助,而兩組之孤獨感越高則較困難於社會關係中找到適當的人尋求建議或發洩情緒。因此,於臨床上可針對不同的功能進行介入,以增進患者社會支持程度。

    Patients with bipolar disorder (BD) have lower social support in both acute and euthymic phase. Social support contains various dimensions, including perceived support (PS) versus actual support (AS), instrumental support (IS) versus emotional support (ES). Social cognitive impairments contribute to inadequate social support in BD patents. Furthermore, psychological factors, such as loneliness are negatively correlated with perceived emotional support. We aimed to explore the association between social cognitive abilities, subjective loneliness and specific aspects of social support in euthymic BD patients. To date, 59 euthymic BD patients and 43 healthy controls were recruited. All subjects completed the social support measures (Measure of Social Function, MSF), social cognitive measures of emotional recognition and theory of mind (ToM), and UCLA loneliness scale. We observed lower social support, poorer ToM and emotional recognition, and higher loneliness in euthymic BD patients. A significantly positive association between ToM and IS was found in both groups. However, no significant association between social support and emotional recognition was observed. On the other hand, higher loneliness was associated with lower ES in both group. These findings may suggest that impairments of social cognition in euthymic BD patients, especially higher-level social cognitive abilities, may contribute to lower social support in tangible assistance (i.e. IS), while higher loneliness contributed to lower social support in emotional needs (i.e. ES).
