  • 背景:機構青少女是危險性行為的高危險群。機構青少女通常成長於失能家庭或曾有受虐、疏忽等受創經驗,這些經驗使其難以培養健康行為,增加發生危險性行為的風險,而導致非預期懷孕及感染性病的機會增加。
    研究目的:本研究以安置機構青少女為研究對象,以性健康議題衛生教育課程為介入措施,探討此課程對改善青少女性健康知識、態度、自我效能及行為意向之成效。
    研究方法:採非對等控制組類實驗設計法,以方便取樣選某兩所安置機構20歲以下之青少女,並依機構分為實驗及對照組。共收集對照組68位與介入組17位受測者,介入組在介入措施前後填寫性健康知識、態度、自我效能及行為意向量表,對照組則在間隔兩週前後填寫量表,兩組皆在三個月後追蹤後測。收集資料並進行描述性統計,以卡方及重複測量變異數分析比較兩組基線、後、追蹤後測之差異。
    研究結果:本研究發現介入組之性健康知識、自我效能在後測及三個月追蹤後測皆顯著進步,對性健康態度、行為意向未達顯著差異,而介入組青少女對介入措施之滿意度高。行為意向的測量發現在對方未強迫、溫柔態度尋求性交時,青少女的意願較被強迫時高。
    結論:此介入措施有效地提升青少女的性健康知識與自我效能,然而,對於態度、行為意向、實際性行為的改變,仍需有更長時間的介入及追蹤研究。介入措施針對性經驗、性虐待等特殊背景的青少女,增加幫助其辨識自我情緒及情感需求之內容。

    Background: Looked after teenage girls are at high risk of sexual risk behaviors. Growing up in a dysfunctional family or having maltreated experience in childhood provides little opportunity for these teenage girls to cultivate healthy sexual attitudes and behaviors. The probability of unintended pregnancy and sexually transmitted infection is increased along with the increase of sexual risk behaviors.
    Purpose: This study aimed to evaluate the effectiveness of a healthy sex education on looked-after teenage girls’ sexual knowledge, attitude, self-efficacy, and sexual intentions.
    Method: A quasi-experimental design with pretest, posttest and 3-month follow up test and a nonequivalent control group was used to evaluate the intervention. Convenience sampling was used to recruit adolescent girls who were under the age of 20 from two halfway houses in Taiwan. All (17 in intervention group, 68 in control group) participants completed baseline, posttest (2 weeks later), and 3-month follow up test questionnaires including sexual knowledge, attitude, self-efficacy, and sexual intention. Data were analyzed using descriptive statistics and repeated measure analysis of variance.
    Result: The results of this study revealed that teenage girls in the intervention group reported higher sexual knowledge and self-efficacy scores in posttest and 3-month follow-up than did teenage girls the control group. No significant difference was found for sexual attitude and sexual intentions between two groups. Most teenage girls in the intervention group were satisfied with healthy sexual program. Girls’ sexual intentions were higher in unforced situation.
    Conclusion: The significance of the present study shows that it is effective to provide sexual health education for looked-after adolescents to improve sexual knowledge and self-efficacy. A long-term intervention which is specific to sexually active and sexual abused teenage girls is needed to increase girls’ self-awareness and emotional need.
