靜脈輸液所引起外滲一直是臨床上常見的意外,是指在輸液期間藥物或溶液從血管外漏到周圍組織所引起的損害,發生的原因具體取決於插管技術、病人的循環狀況以及藥物屬性。由於輸液的過程很長,醫護人員沒辦法隨時監控輸液狀態,肉眼也較難辨識外滲的早期徵兆,通常發現時已經造成嚴重的傷害,導致延長治療的時間,造成醫療糾紛和聲譽損失,因此建構一套完善的監控系統,不僅能有效的降低長期照護的醫療成本,同時也能提升病患的照護品質。而現有的研究多數針對技術層面,較缺乏使用性與用戶體驗的相關研究,因此本研究的目的是設計一套外滲偵測系統,藉由光學傳感器來偵測外滲,結合止流閥的功能以減少傷害,並透過Wi-Fi傳輸訊號實現遠端監控。在開發前期,透過啟發式評估來了解用戶需求,並進行使用性測試。測試後以系統可用性量表(SUS)及半結構式訪談評估整體的使用性及接受度,並根據實驗結果給予改良,結論中對於系統的設計策略給出一些建議供後續的研究參考。
Extravasation caused by intravenous infusion is a common clinical accident. Extravasation is the injury caused by the leakage of drugs from blood vessels to surrounding tissues during infusion. The cause of extravasation depends on the cannulation technique, a patient's circulatory condition, and the characteristic of drugs. As the infusion process is long, caregivers cannot continuously monitor the infusion state. Besides, it is difficult to visually recognize the early signs of extravasation. Typically, severe extravasation has already occurred before it is detected. This leads to prolonged treatment time and medical disputes. Therefore, a complete extravasation detection system can effectively reduce medical costs and improve the quality of health care. However, numerous studies focus on technical levels and clinical evidence. Only a few studies focus on usability and the integration of hardware and software. This study aims to design an extravasation detection system that uses an optical sensor. A cutoff valve is used to reduce injuries, and remote monitoring is realized through wireless transmission. Heuristic evaluation is employed to understand user requirements and conduct usability testing in the early development stage. The system usability scale and a semistructured interview are adopted to evaluate the overall usability. The results of this study provide a guide for the design of extravasation detection systems in the future.
