The study aimed to establish the reliability and validity of the Taiwanese version of the Activity Card Sort (ACS-TW), a culturally specific tool designed for assessing activity participation in older adults. Engagement in activities is crucial for older adults' well-being, influencing their physical and mental health, and overall life satisfaction. Researchers recruited 219 healthy older adults from community centers in southern Taiwan. Participants were assessed using the ACS-TW and other tools, including the Taiwan version of the Meaningful Activity Participation Assessment (MAPA), the age version of World Health Organization Quality of Life questionnaires (WHOQOL-AGE), Integrated Care for Older People Screening Tool-cognition part (ICOPE), and frail test (SOF). Psychometric properties such as internal consistency, test-retest reliability, concurrent, convergent, and discriminant validity were evaluated, and regression analysis was performed to investigate associated factors. Results showed high internal consistency (α = 0.939, p < 0.001) and excellent test-retest reliability (intraclass correlation coefficient = 0.931, p < 0.001) for the ACS-TW. The tool demonstrated significant correlations supporting its concurrent (r = 0.536, p < 0.001) and convergent validity (r = 0.222, p < 0.001). Discriminant validity was also confirmed by distinguishing activity participation levels among older adults with differing health statuses (t = -4.30, p < 0.001). Factors significantly associated with activity participation included age, gender, health status, and education level (R2 = .22). In conclusion, the ACS-TW is a reliable and valid instrument that effectively reflects activity participation and changes in community-dwelling older adults. It is suitable for various practical settings like community health promotion and long-term care services. Future research should broaden the sample to include diverse populations to enhance generalizability and local applicability.
